Genesis of the Daleks - S12-E4
Deliberate mistake: Chief of Security Nyder wears an iron cross on his uniform which mysteriously disappears in episode 3 - apparently the producers felt it was too close a reference to Nazism so it was removed from the show. (Source: DVD commentary).

Revealing mistake: In episode 6, when the Brigade Leader shoots the Primord Stahlmann, he falls down but no bullet holes are made in his costume. Thanks to Producer Barry Letts for spotting this one on the DVD commentary. (00:17:00)

Other mistake: A few times during the whole show there are certain shots where the corners of the screen are blacked out, as if a circular lens cap has been fitted to the camera. There seems to be no logical reason for this, therefore it has to be a mistake. One example is when Jonas is leaving his wife for the last time and he says 'there's a big battle brewing'. (00:07:25)

Continuity mistake: In episode 1, when the Doctor opens the garage doors with his 'door handle', note the two men in the background, the position of the car behind him and the white pillar. In the next shot, the men and pillar disappear and the car moves to a different spot. In the shot after that, the tanker truck is now parked in front of a pillar. You can see near the beginning of episode 3 that there are two entirely different buildings that the tanker had been filmed in front of. (00:14:25)

Continuity mistake: In episode 4, the Doctor's bow shrinks from when he talks to Liz Shaw while in prison, to when he stows aboard a jeep later. In the DVD commentary, producer Barry Letts blames himself for this mistake. (00:13:00 - 00:19:50)

Continuity mistake: Before they attempt to trap König with the raised helmet trick, we see one of them light a cigarette. It initially appears to be very large and misshapen. When the angle changes, the cigarette has shrunk to more than half its previous size and is perfectly rolled.

Continuity mistake: When Frank is first descending in the elevator, he is tapping his foot to the beat. In the close-up, he seems to be just barely lifting his heel just a few inches from his other foot, but on the wider shots, his foot is lifting MUCH higher and about a foot apart. (00:24:05)

Continuity mistake: When Frank-N-Furter is talking to Dr. Scott just before he reveals his relation to Brad, Furter's right leg goes from being on the arm of the chair Dr. Scott is sitting in to a standing position from shot to shot. (01:01:50)

Continuity mistake: When the Giant Yakuza has finished frisking Bill Jenkins, his black car is seen behind him. When he pulls out his gun and shoots into the air, however, it has moved much further away. Or to be precise, the camera and the giant have moved further away from the car. (00:56:55)

Visible crew/equipment: During the "target acquired" chase scene when the red pickup truck flips over after going through the split semi-trailer there is an overhead shot shown of the action. The next shot when the T-X looks in her rear view mirror at the action, the camera crane that filmed the previous overhead shot is visible in the background. (00:32:25)

Continuity mistake: When John and the vet arrive at the mountain in their Cessna, the sun casts a shadow straight under the plane. The very moment they walk through the door into the hangar integrated in this mountain (which in the film is immediately after they landed), their shadows are in front of them indicating a low sun. (01:30:45)

Continuity mistake: In the escalator scene when Arnie uses a corpse as a shield, there's a close-up of Arnie's gun firing, and we can see there's blood on the side of the corpse's head. However, in the wider shots there isn't. (00:31:00)
