Continuity mistake: When the lift malfunctions, the lady has her head and arm outside the lift when she starts screaming to pull her back inside. We get one shot of her again with her head and arm outside before the shot changes to the security camera angle, and in this shot only her head is sticking out from the doors, with no time for her to move her arm (which was stuck) back inside the lift car. Continuity mistake: When Rain and JD are tasked with watching the prisoner in Dining Room B, Rain's hair on her right-hand side drops down and curls to the front. The camera changes and now it's curling to the back. (00:27:30)

Continuity mistake: In the laser corridor, the team leader is stood upright when the lights comes on, but as the camera changes to a front view, he is crouched. (00:30:10)
Suggested correction: It's easy to miss, but there is a quick insert-shot in-between these two shots that shows the team leader crouching down suddenly. (The camera is focused on the back of his head as he ducks down suddenly.) It can be seen in this video (https://www.youtube.com/watch?v=rOLe15-5Xhw&t=65s) around the 1-minute mark.

Other mistake: During the scene with the laser, the laser moves along the floor where the EMP device is located, several times. However, when Kaplan and Alice enter the room, the bag with the EMP unit is still intact. (00:31:15)

Deliberate mistake: When the woman is attempting to escape from the elevator, it dislodges and falls down, so then the actress is facing the ground...but when her arm hits the floor, the floor itself bends. If you watch the DVD with the commentary on, the director points out that it was a break away floor in case the actress' head accidentally snapped forward, to avoid any injury. (00:08:00)

Revealing mistake: When the team leader is opening the door to the Red Queen chamber, he clips an electronic device to the door to hack the code. The device is actually a blood glucose monitor as used by diabetics - an Accu-Chek, I think. (00:31:05)

Trivia: All the cast and crew (including the producer) had to undergo a grueling one week of military training (which they dubbed their 'week of hell') under the supervision of a battle hardened ex-navy SEAL. By the end most people were naturally eager to give their trainer a little of his own medicine. Miss Jovovich (Alice) did: he's the zombie she head kicks into the test-tube cabinet and then steals the gun from (before fighting the zombie dogs). What sweet justice.
