Corrected entry: During the fight at the river pirates land base, Jimmy Stewart throws a small keg of gunpowder at a fire. Seconds later there is a very large explosion. Frame by frame slow motion reveals that the keg hits the log behind the fire and totally shatters spilling its contents over the fire and nearly putting it out. Had the keg contained gunpowder it would have instantly caught fire.

The Fly (1986)
Corrected entry: After Seth Brundle has gone through his final stage of metamorphosising into the Brundlefly, he throws Ronnie (Geena Davis) into one telepod and then starts to make his way across the lab towards the other one. Watch as he emerges from behind a wooden pillar, there's one shot where a crew member's arm is seen reaching inside the back of the Brundlefly puppet, to move the creature's head. You'll need to slow-mo to see it but it's there. (As per the rules of this site, visible crew members is an acceptable use of slow motion.)

Sex and the City (1998)
An american girl in paris (part deux) - S6-E20
Continuity mistake: In the first shot of the diner as the girls are waiting for Carrie to arrive, the baby photo of Charlotte's daughter is not visible on the table. When Carrie comes in, there is a clear shot of the entire table, and the baby photo is still nowhere in sight. But in the shot where Carrie sits down at the table with the girls, the baby photo is suddenly sitting upright on the table near Charlotte, and fully visible. There was no time for this to happen, as they are just sitting down. Carrie is still hugging her friends (Charlotte) at the table. (00:41:40)
Suggested correction: The scene doesn't take place in real time. When they hug each other at the door the food has not been served. After the hugging it cuts to all four seated and the breakfast already on the plates, with the picture in sight. Therefore, they had plenty of time to see the menu, order, wait for their food and, of course, place the picture on the table.

An american girl in paris (part une) - S6-E19
Continuity mistake: In one of the deleted scenes for this episode, Carrie has to take her laptop out of her purse in order to make room for the garment. She throws the laptop on the bed, with the 'Apple' symbol facing down. But just after that, the laptop is shown lying on the bed with the 'Apple' symbol facing up. To my knowledge, there is no 'Apple' logo on the bottom of the laptop.

I heart ny - S4-E18
Continuity mistake: When the horse drawn buggy is pulling over to the curb in front of the hospital, you can see it dodging traffic and cars. But after Carrie leaves and the buggy pulls away with only Mr. Big, suddenly there are no cars or traffic in sight. The entire street from that direction appears to be completely empty. (00:22:50)

Sex and the City (1998)
Continuity mistake: Right before the '3-way'; Samantha takes off the top of her lingerie/dress and pushes it down to her waist, to reveal a strapless black bra she is wearing underneath. She does this in response to the younger girl removing her top, and they are both obviously getting ready for the main event. But in the next shot when Samantha is kissing Richard in bed, she is suddenly wearing the black lingerie/dress pulled back up over her shoulders again. Why would she put the dress back on, after removing it for sex? (00:23:10)

Sex and the City (1998)
Continuity mistake: When Mr. Big comes up to the cabin to talk to Carrie, he isn't wearing glasses. After he drinks too much wine, they decide he has to stay the night at the cabin. Aiden throws him a pillow and blanket. Right when the blanket hits Mr. Big as he is sitting in the chair, you can suddenly see what look like some sort of metal frame glasses hanging vertically/falling off from Mr. Big's face, as if they were knocked off by the blanket. Where did the glasses come from? He wasn't wearing them just before that. (00:24:40)

Ghost town - S4-E5
Revealing mistake: When Samantha and Maria are in the bath tub arguing; Samantha decides to leave, and suddenly gets up out of the water. She is not wearing anything from the waist up; but if you look closely just at the end of the scene, you can briefly see that Kim Cattrall is actually wearing some sort of minimal dark undergarment below her waist to cover herself. You can see a black strip of material across her hips. (00:11:20)

Sex and the City (1998)
Corrected entry: At the beginning of this episode, Carrie mentions in voiceover that they had been in LA for over a week. It's obvious in this episode, that they are staying in LA for a second week. But Carrie never showed up at the second meeting for the Production Company, which would have ended her work with them. Since the Production Company was paying for 2 hotel rooms; it isn't likely they would have let the girls stay in those rooms for 2 weeks for free, when Carrie quit the project after only a few days. And it's doubtful the girls would have elected to pay full price for an expensive hotel in LA for almost 2 weeks on their own, when they had expected to stay there for free.
Correction: Agree with the other corrections, but would add that Carrie wasn't working for the production company. She was invited to California to discuss optioning her column for a movie. Regardless of whether or not she attended a second meeting, she was their invited guest and the arrangement included a two-week hotel stay, which would be pre-paid. The remainder of a two-week hotel bill is a small business expense that the production company could write off as a tax deduction. Suddenly ejecting her from the hotel would be unprofessional, cheap, and end possible future collaborations and is something Carrie would likely write about in her column.

Sex and the City (1998)
Continuity mistake: During this scene when Samantha is confronting her new assistant, the office set-up is completely different than before, when Nina G. was working for her. The assistant office space is much larger, glass desk, different angle, next to small conference room instead of Samantha's office, etc.; obviously not the same office set as previously used. Yet Samantha has not changed offices at all during this time period, as it wasn't too long ago when she fired Nina G. Something as major as changing offices would definitely have been mentioned in the plot (as changing apartments always is); and there hasn't been enough time since she fired Nina G. (00:13:45)
Suggested correction: You're assuming that moving to a new office must be a plot for a new episode, which makes no sense (plus, it's a show that revolves around sex, women, restaurants and bars). Furthermore, the episode you mention is S2E17. Many months have passed and it's perfectly plausible for Sam to move some place else.

Monty Python's Life of Brian (1979)
Factual error: There were no gourds in Ancient Judea, of that type. The 'gourd' mentioned in the Bible refers to a colocynth plant, which yields only a small fruit, 5 - 10cm diameter. The gourd in the film is from a pumpkin, and not from a watermelon, as others have suggested. Pumpkins originated in the Americas, and there weren't any in the rest of the world before Columbus.
Suggested correction: This is a movie where Brian falls inside a flying UFO with alien pilots. I seriously doubt that the filmmakers had any intention whatsoever to be historically accurate.
Regardless of the random UFO scene, this is still a factual mistake. It doesn't matter if a film maker sets out to intentionally make a historically accurate film or not. There's nothing to suggest this film was set in an alternate past or that it was a sight gag as if it was a Mel Brook's film.

Conan the Barbarian (1982)
Continuity mistake: In the scene when Conan's mother is killed, we see her using both hands to hold her sword toward Thulsa Doom and his men. The camera cuts to young Conan and he is holding her right hand. When the camera cuts back to her, she is holding the sword with both hands again.

Dirty Dancing (1987)
Corrected entry: In the scene where Johnny and Baby are in the dance studio, Johnny puts the records down twice, once when he agrees to do the dance and the second time when Neil is saying to Baby, "He's hard to talk to but the ladies seem to like him" (you see him do this in the mirror) and then we see him slam the lid to the record player down, which scares Baby.
Correction: Johnny has several records in his hand. When he puts the first one away he still has some in his hand.
Correction: The original mistake is right. He stops the record from playing. Removes it, holds it with one hand, turns sideways and bends down to store it. Then his hands are empty. Angle changes and the record is back in his hand and he's in the first position repeating all of the movements.

Indiana Jones and the Temple of Doom (1984)

Visible crew/equipment: After Willie spits in Indy's face it cuts to Short Round running through the mine, and just as Shorty turns his head toward the camera behind him, we can catch a glimpse of something blue which does not belong in this dark dreary environment, at the right side of the screen. There is a tall spotlight and two crew members, one is wearing a solid blue shirt and a cap, and the other a blue/white striped shirt with white pants. (01:21:25)
Suggested correction: This takes place in milliseconds and there's no way to see it unless you pause the image or play it in slo-mo. It should be under trivia.
Right now I have it playing on Netflix, and there is no slow motion. The striped blue shirt is noticeable as it plays without any slo-mo. If something catches my eye onscreen, it is fair game to go back and rewatch what was noticed in the first place. And in order for me to confirm what I find, I do pause, and may take screenshots. By the way, I've seen this movie countless times over the years, and it was just a matter of catching that glimpse of the color blue because it popped out against the dreary surroundings. I'll leave it up to Jon whether to move it to trivia or not.

Superman II (1980)
Corrected entry: In Richard Donner's cut, after Superman saves the kid in Canada he speaks with Lois and his hair keeps changing from dry then wet then dry back again between shots.
Correction: Richard Donner's is not a finished movie, but an experiment of how S2 would have looked like had he finished it. It is made with material from different sources: Richard Lester's film, auditions, previously unreleased scenes, newly filmed stuff... Thus, it is impossible for it to have any continuity whatsoever and was never intended to have any.
