Revealing mistake: When Max is strapped to the armchair it becomes obvious on at least three occasions throughout the scene that the strap is not secure and he can escape the chair by pressing down into the fabric, but chooses not to.

Question: Even despite the extreme circumstances as presented in this movie, does any producer or director of any live show have any real control over whether a network goes to commercial or not?
Chosen answer: Yes. Any live show can go to commercial at any time. It's a way to have a check against things going wrong.

Continuity mistake: When the kids jump in the shower towards the end of the episode, they're all wearing footwear. When the shower is turned on and we see a wide shot immediately after the kids get wet, the boy is now barefoot and there is no sign of his trainers anywhere in the immediate area.

I seem to remember a TV show, probably an episode from an Australian children's series aired in the UK sometime in the mid 1990s, but I would be grateful if anybody could shed any light on this, or at the very least identify which TV show it came from. In the episode in question there is a boy who, for whatever reason, is chained to a supermarket trolley that obviously follows him around. Said boy and trolley somehow end up in the sea and another kid's mother swims out with some cutters to cut the chained boy from "his" trolley before he drowns.
Chosen answer: It may be part of the Paul Jennings Weird Storie episodes, also famous for the Aussie Series Around the Twist.

Gourmet Night (a.k.a. Gourmet's Paradise) - S1-E5
Visible crew/equipment: At the start of the episode, when Basil follows Manuel into the hotel, the shadow of a crew member holding a boom mic can be seen bottom right as the camera follows them across the terrain. (00:01:30)

Continuity mistake: Where Cody is watching the tape for the first time and Cindy rushes in, Cody can be seen wearing a greyish washed-out looking jumper. When we next see Cody in a couple of cuts time, his jumper is now a brighter shade of blue. (00:45:00)

Visible crew/equipment: When Mr Bean parks his car outside the examination hall, as he reverses into his spot, you can see a group of plain-clothed crew members pushing the camera reflected in the car nearest to the camera. More noticeable as the camera is actually moving. (00:00:35)

Hair By Mr. Bean of London - S1-E19
Continuity mistake: In the scene at the country fair, Mr Bean interrupts a boy doing the buzzer game. When Mr Bean is on his second go at this puzzle and stands up after unplugging the device, you can see the same boy come round a table behind Mr Bean. Cut to the next shot, the boy has disappeared from where he should be behind Mr Bean and can now be seen walking across the back of the hall.

Hair By Mr. Bean of London - S1-E19
Continuity mistake: At the country fair and during the pet show segment, when the boy in the blue T-shirt is putting his dog into position for the obedience test, he can be seen standing behind the dog as we look at it, but on the next camera cut, he's standing in front of the dog.

The Kipper and the Corpse (a.k.a. Death) - S2-E4
Revealing mistake: Just after Polly comes down to to the kitchen to tell Sybil that Mr Leeman's dead and they leave, Basil wanders back in, to learn the news. Watch the wall on the right on the staircase when Basil shoves past Sybil on the stairs. The wall shakes so much at that point it's virtually impossible to miss.

The Builders - S1-E2
Continuity mistake: Towards the end of the episode, a couple of scenes before Mr Stubbs arrives, we see Basil start up the tape player prior to hiding in the kitchen. The garden gnome on the desk is right next to the tape player at this point. When Sybil turns the tape off the player has rotated 45 degrees or so (despite nobody else having come into reception in the intervening time) and the gnome is now further along the desk towards the camera as we look on the wide shot. The gnome in the next shot is seen beside the checking-in book when Basil wanders out of and back into the office a couple of scenes later, yet when Mr Stubbs is asking about the lintel for the new door shortly after that and we see Sybil writing in the book, the gnome has moved out of shot altogether.

Visible crew/equipment: Just after Kevin locks Prescott in the freezer, the shot tightens up on the window and you can see a slightly distorted reflection of the cameraperson in the door below the window. It's quite hard to miss as the shot tightens.
16th Oct 2004
Visible crew/equipment: When Mr Bean is waiting for his ticket number to come up, we see a schoolboy and his mother come in with a saucepan stuck on the boy's head. When the shot is framed to show Mr Bean and the boy, if you look closely you can just about make out the (slightly distorted) reflection of a camera tripod on the saucepan. (00:04:40)
9th Oct 2004
Continuity mistake: At the very end of the movie after the wedding, Cindy and the other guy drive off, leaving Cody behind. We see the boy running after them along a straight road. After Cindy stops short of striking the child, Cody is then hit by another car coming from his right hand side and they're suddenly at a crossroads. They weren't at the crossroads a couple of seconds earlier. (01:11:30)

Chosen answer: This is the very reason my brother and I used to jokingly call the show, "Murder, She Caused." It's amazing she was ever on anyone's guest list for a party, given the likelihood someone would end up deceased. As to your question, most of the time, Jessica Fletcher would have had an air-tight alibi, as she was in a room full of people, or her whereabouts were accounted for when a murder occurred elsewhere. It also seems to me that there were episodes where she, purely with respect to opportunity, could have been a suspect. I believe she even acknowledged that as a logical possibility from time to time, even though she knew, of course, she was not the killer. However, the investigation would obviously rule out the possibility of her involvement, eventually.
