Continuity mistake: The Apocalypse/Mayhem: Just before the first battle with Straga, you walk down a street where there is a large hole in the road filled with lava and some Angels are fighting some Minion Demons. On the other side of the hole, there is nobody. But once the cut scene starts on an angel in the sky, he swoops down towards that spot and suddenly Abaddon and another Angel Soldier appear standing on the far end of the hole.
Other mistake: The Apocalypse/Mayhem: Right as the cut scene starts where you meet Abaddon, there are three Angels on screen. The one standing on a building is just looking around, and right before the one hovering by the camera starts to swoop down, the one standing on the building turns his staff towards a diction, charges a shot and fires... Only he's shooting at nothing for no apparent reason. There are no demons or humans visible that direction, and only distant buildings down the street far out of his range. So he's just animated to shoot randomly down the street at nothing.
Other mistake: The Apocalypses/Mayhem: If before heading down the road where you meet Straga, you pick up a car and then go down there, the cut scene introducing Abaddon and Uriel still plays. War looks normal walking up and talking with nothing in his hand. But as soon as the cut scene ends, you will be standing there now holding the car above War's head once more. And considering that right after this cut scene, you have your first encounter with Straga, and his weakness at this point is throwing cars at him, this allows you to get an early hit on him right off the bat by throwing the car as soon as the cut scene ends, before he ever hits the ground to cause cars to rain from the sky for you to use.
Revealing mistake: The Apocalypse/Mayhem: During this level, meteors are constantly falling down to the earth. A lot of items in the environment are destructible, including all vehicles seen. However, if a meteor strikes a vehicle, like a bus, it does nothing to it. But rather passes right through the bus and just leaves an impact crater under it.
Bug: The Apocalypses/Mayhem: After defeating the first group of Angels, a Gatekeeper will rise from the ground and start stomping down the street. As you follow, a short cut scene happens where War loses his power at the sound of a horn blowing. Sometimes at that point, as the Gatekeeper passes some rubble a human will be stuck up in the air. Hovering and running around in circles just up in the sky.
Continuity mistake: Opening Cut Scene: When the civilians are gathering around a crater to see what the meteor was that just hit, one of them is a guy in a red hat who is wearing a vest or coat with long sleeves of a dark color. When the Trauma Demon emerges and turns towards them to roar, the red hat guy is seen starting to turn to run away in a panic. Only now he's wearing a blue hoodie with white sleeves.
Audio problem: The Choking Grounds: After being freed, the Gatekeeper will walk down the road and then leap up onto a higher area and proceed to walk behind a building to go away. Each step he takes is very loud as he's a giant rock creature on a paved road. But the moment he passes behind the building, the loud sounds of his steps stop where as you should still be able to hear him walking a way for a good distance. If you're standing in the right spot, you can even see him just suddenly despawn behind that building.
Continuity mistake: The Choking Grounds: Once freed, the Gatekeeper starts walking towards War and speaks "Ancestors praise you." As he says this, he almost right to War, but the camera cuts to a different angle before he finishes the last word and suddenly the Gatekeeper is several feet behind War. It seems the animators had him jump forward to keep from showing the Gatekeeper stepping on War.
Revealing mistake: The Choking Grounds: After defeating the four arena challenges in the Realm of Shadows, and going back to Gatekeeper and summon him up to free him. There is a tree to his left, on War's right, that has low hanging branches and a lantern hanging from one branch. As the Gatekeeper is standing up, his left arm passes through the branch and lantern. This is a being made of solid rock and the tree and lantern phase through it's arm like a ghost. His right arm also passes through some of the leaves on the right side of him.
Continuity mistake: After completing the Abysmal Armor and then beating the game in one play through, you can start a new game but choose to start off with the Abysmal Armor on from the start now. However, while you are wearing the Armor in regular game play, as soon as the game starts any cut scenes War will still look normal in his regular attire, the Abysmal Armor vanishing until game play resumes.
Revealing mistake: Scalding Gallows: There is lava all around Samael's prison down in a chasm. Jumping down into it triggers the lava death animation with fire all around War. However if you jump towards the secret chest and miss, landing on the stone face of the wall you still die. Even though you don't actually touch the lava yet, it still triggers the lava death animation.
Bug: The Ashlands: Down in the room where you can get the Strife's Offering. There is a movable platform with a gradable ledge on one side. If you move this platform to one of the corners of the accessible area where their is some rubble and an invisible wall, you can still grab a hold of the gradable ledge on the one side. If you slide over to the far right of this edge and jump off into the corner beside the platform, you will get stuck behind the invisible wall and be unable to get out. This becomes a Soft Lock bug as there is no possible way to get out of this spot once you're stuck and you must reload your save file.
Bug: The Ashlands: There is a hole in the area where the Abysmal Riders spawn. This hole is like a well with wooden structures around it and sticking out towards the sand. With the clunky controls of the game for when riding on Ruin (your horse), it's easy for you to get stuck on this structure. If you jump on Ruin while moving towards it, you can get stuck and Ruin will just be there running in place on this board until you get off him.
Bug: The Ashlands: There is a hole in the ground towards the back with the climbable wall that leads down into the water below. There is a bug that causes the Abysmal Riders to spawn a new set every time you climb back up out of that hole. So each time you go down and climb back up, 2 to 4 of these enemies will spawn out in the desert close by and start riding around in circles. You can continue to jump down the hole and climb back out causing more and more to spawn creating an army of Abysmal Riders out there which you can then lead into a cave to farm XP and souls off.
