Continuity mistake: When Trinity confronts the agent on the rooftop and says, "Dodge this", she points her gun to temple of the agent while his head is turned to her. He glances back, but never fully turns his head. The instant next shot shows him being shot facing Trinity with the gun in the middle of his forehead. (01:42:20)

Suggested correction: Actually, I think that's correct and reflects how the agent 'took control' of his body. When he took on the pilot's form the pilot turns to the side in agony but the agents face actually appears from the side of his skull. So being shot in the temple then, would be consistent with a shot either to the front or the back of the head. In considering that the victim turn his head to the right when he was 'transformed' and given that agent was shot in the left temple, it would imply that the victim was actually shot in the back of the head? The would we saw should've been an exit wound, but it appeared to be an entry wound.

Not at all, all this occurs after the takeover has happened. The screenshot is pretty clear. He's facing forwards, gun at his temple, shot cuts and he's instantly facing Trinity, gun to his forehead.

Revealing mistake: In the government lobby scene, when Trinity kicks the shotgun out of the guard's hands, watch closely, the man actually throws the shotgun over his own shoulder. It's very well timed, but if you watch closely you should be able to spot it. (01:42:30)

Visible crew/equipment: When Trinity asks Neo 'How did you do that?' after he's dodged the bullets the white reflective screen is visible in the left lens of her glasses. (01:42:35)

Factual error: During the lobby shootout, Neo pulls out a pair of VZ-61 Skorpion machine pistols and runs down the lobby blowing away the guards. While he is running, we see the blank shells falling to the ground beneath him. However, the shells are 5.56mm NATO, which is a cartridge that is too long to fit in this gun. (01:42:45)

Other mistake: When Neo shoots with the chopper's machine gun the window smashes from a single point in the middle, shattering two panes. Water then starts splashing up from bullet impacts right at the base of the window, but the glass doesn't smash anywhere around there - it could only work like that if Neo was shooting down at an incredibly sharp angle, which he obviously isn't. Continuity mistake: During the lobby shoot out Neo does a cartwheel to pick up a gun on the floor. If you look closely at the wide shoot, his hand grazes the gun and starts to lift off the ground without it. In the close up shot imediately after, he picks up the gun near the trigger. (01:43:22)

Continuity mistake: Near the end of the film we see Neo in the chopper firing the mini gun through the window at the agents. In one particular shot note the metal chair pulled out from the desk which falls over after being shot at, then a few shots later is suddenly back upright again. (01:43:30)

Factual error: When Neo is shooting thousands of bullets out of the Gatling Minigun from the helicopter, there is a shot of "cases" and links falling from the machine gun onto the helicopter floor. Only those "cases" are complete rounds, bullet and all (or possibly blanks) instead of just empty, spent cases. (01:43:45)

Continuity mistake: After filling a room full of holes with the machine gun from the helicopter, the Agents abandon their current bodies and take control of ones that were standing outside the door to the room Morpheus was held in. Where were these guys the whole time the gunfire was happening in the next room? The Agents were standing in relaxed positions as soon as they took control. Wake up guys. (01:44:10)

Continuity mistake: Throughout the "lobby scene" the walls and pillars of the lobby are shown being torn apart by bullets. At the end of the scene, Neo and Trinity proceed into the elevator and the overall damage is apparent. They set the bomb, get out of the elevator and the bomb and elevator drop. When the explosion is exiting the elevator shaft, the lobby is spotless... no SWAT men, no guns, and perfect pillars and walls, not a bullet hole in sight. It could be said that the dead guards are on the sides of the room, invisible from the camera's angle... but what about the last guy Neo kills right in front of the elevator? (01:44:10 - 01:45:30)

Continuity mistake: Neo and Trinity kill all those soldiers on the roof before the agent comes to fight them. The agent shoots at Neo and during the whole dodging bullets scene every single corpse is gone. More to the point, Trinity's disappeared too! (01:45:36)

Continuity mistake: When on the roof, Neo fires at the enemy agent, who dodges all his bullets, but the windows of the building behind him are undamaged. (01:45:36)

Revealing mistake: During the rooftop rescue, when Morpheus is hanging from Neo's arm (who is hanging from the helicopter), a safety wire is visible running out of Morpheus' sleeve and into Neo's hand. (01:45:48)

Continuity mistake: When Neo has just jumped from the helicopter, he is left standing with the safety rope next to his right foot. He then slings it round his right arm, and he's looking at the rope on the floor, which has moved next to his left foot. (01:46:05)

Continuity mistake: When Trinity and Neo have rescued Morpheus and are flying through the air, Neo drops Morpheus then Neo lands, but Neo is halfway on the rooftop, yet when he grabs hold of the rope and is dragged along the rooftop, he is sliding almost the entire length of the rooftop. (01:46:05)

Deliberate mistake: In the rooftop fight scene where Neo unloads 2 pistols at an agent and misses, you can hear the guns firing from behind, followed by 2 clicks suggesting that the clips are empty. Neo's guns are both semi-automatics and would therefore not click when empty as the slides would be locked back after the final shot. The click sounds were obviously added later to indicate to viewers that his clips were empty. (01:46:15)

Continuity mistake: When Neo unloads two 9mm Berettas at an agent who dodges all the shots, right after he runs out of ammo, the camera is facing Neo dead on and the recoil slides on the pistols are closed. The next shot where he drops them and says, "Trinity... help." the slides are locked back with the ejection port open. (01:46:20)

Factual error: When Agent Brown shoots at Neo on the skyscraper, the last shot scrapes Neo in the knee and on his shoulder. This would cause both shots to be vertical when standing up. However, when Neo stands up and indicates to the scrape, the shoulder scrape is horizontal - not possible given the trajectory Agent Brown is firing from. (01:46:30)

Continuity mistake: During the rooftop scene check out the tiling situation. When Neo takes two bullets, he falls to the ground. He lies mainly inside one tile, even the left gun fits in. (And it's on a vertical line) His top hand lies just on the line. Now, watch again when the agent is dead. All of the sudden he's in-between two tiles, and the gun is now on a horizontal line, but not a vertical. (01:46:36)

Continuity mistake: After the helicopter smashes into the building and Neo saves Trinity, they look at each other and you can see smoke from the building. After that, Morpheus is talking to Neo. You can see several shots of him and Neo talking and behind them, the building and its windows are still intact. (01:46:50)

Trivia: It's been said that Neo is an anagram for "one". I think it's more important to realise that Neo means "new". His real name is Thomas Anderson. He is called Thomas at the beginning of the movie, when he doubts the truth -- that the world as he knows it is not real. Thomas is the "doubting" disciple in the Bible.
