Continuity mistake: Just after Ricky has broken the assistant warden's desk and dodged a bullet fired at him, there is a shot of Ricky wiping off a small amount of blood on his face, tasting it, then clenches his fists and advances on the assistant warden. Here we can see a small droplet of blood on his stomach. But in the next shot, when he stops charging toward in the assistant warden, the blood on his stomach has vanished. (00:27:00)
Continuity mistake: When Ricky begins walking out of the assistant warden's office after first meeting him, he holds up a photo of his girlfriend and tells him to keep his nose out of his affairs. Some blood can be seen on the back of the photo. In the very next scene, Ricky is shown sitting in his cell looking at the photo, with no blood on the back of it. (00:27:24)
Visible crew/equipment: When the bald inmate has finished on the toilet, he stands up, begins to pull up his trousers and tie them up. Whilst he does this, the camera begins to rotate around the stall. A few crew members can be seen reflected on the toilet stall door when the camera rotates around it. (00:29:45)
Continuity mistake: When the bald inmate is being followed by the other inmates, he begins to walk up a flight of stairs. In the first shot, no-one else is shown on the staircase, but in the next shot, an inmate has suddenly appeared to push him over the banister. (00:30:16)
Continuity mistake: When the bald inmate is pushed over the staircase and cornered by the other inmates, he says "Hold it! What's this about" and is pointing with his right hand finger before moving his hand towards the ground. The camera changes angles and his right hand is suddenly reaching out again. (00:30:24)
Other mistake: When Hai is speaking with Kuang just after stopping him from killing the bald inmate, Hai asks Kuang if he kills him, what do they say to the assistant warden. There is an error involving the English subtitles regarding Kuang's next line. Kuang's line, in the subtitles, is listed across two rows, but the rows are displayed in the wrong order. The first row reads "him, we won't take it anymore." And the second row reads "Hai, lots of men have died because of", which makes no sense when it's all put together. The full line should have been displayed as "Hai, lots of men have died because of him, we won't take it anymore." But it has incorrectly been displayed as "him, we won't take it anymore. Hai, lots of men have died because of." (This depends on which format and which studio distribution you are watching, but it can definitely be seen on the Region B Blu-Ray distributed by MediumRare). Continuity mistake: Just after Hai has stopped Kuang from killing the bald inmate, Hai leans on the staircase behind him. His elbow is shown leaning on the step at first, but in the next shot, his elbow is suddenly leaning along the pole of the banister. (00:31:40)
Continuity mistake: When Hai stabs Kuang against the side of his face, blood is shown slowly poring out of the wound on Kuang's face. The camera then cuts to focus on Hai, and the blood coming out of Kuang's wound is suddenly spurting much quicker. (00:32:32)
Continuity mistake: When the assistant warden throws his glass at the window, a couple of the blinds get covered in juice. In the next shot, much more of the blinds are suddenly covered in juice. (00:33:28)
Continuity mistake: When the assistant warden begins shaking the mints out of his eye, in the close-up shot of his hand, there are about four mints in his hand. But in the next shot, there are suddenly many more mints in his hand. (00:33:59)
Revealing mistake: When Hai jumps down from the cross, Kuang's body, that has been attached to the cross, is a dummy and not a human body. (00:34:37)
Revealing mistake: When Ricky hits the back of Hai's head, one of Hai's eyes blasts out of the eye socket. In the close-up shot of the eye falling out, Hai has been replaced with a dummy. (00:36:54)
Continuity mistake: When Hai stabs himself, blood is shown across his abdomen, but in the shot where he begins to remove his intestines from his stomach, there is no blood shown on the part of his abdomen around his stomach. (00:37:32)
Revealing mistake: In the close-up shot of Hai's stomach when he removes his intestines, it is apparent that the blood that was used is not real blood, but in fact red-coloured water. The blood shown here does not have the thickness that real blood has and drips way too much like water. (00:37:41)
Continuity mistake: When Hai falls to the ground after Ricky punches him, one of the intestines he used to strangle Ricky with can still be seen in his stomach when he falls over. When he gets up, the intestine in his stomach is no longer there. (00:37:51)
Continuity mistake: When Ricky punches Hai whilst he attempts to choke him with his intestines, Hai lands on the ground a foot or two away from Ricky. Hai then stands up, and he and Ricky are suddenly meters apart. Ricky cannot be seen on-screen, but a couple meters of ground can be seen in front of Hai, so the distance between the two has changed. Continuity mistake: When Hai attempts to choke Ricky with his intestines, Ricky punches him and Hai falls back. Ricky is shown with his left fist extended further than his right fist. The shot then cuts to a different angle of Ricky where it is now his right fist that is extended further than his left. (00:37:51)
Revealing mistake: Just as Ricky goes to lift up Hai, a wire can be seen attached to Hai to help lift him off the ground. (00:38:00)
Revealing mistake: When Ricky punches Hai after throwing him into the air, there is an X-ray image shown of Hai getting his skull pummelled in. In the shot after this, Hai can be seen swinging backwards unnaturally, revealing that he is being held up by a wire off-screen. (00:38:05)
Revealing mistake: When the inmates begin to confront the assistant warden after Kuang's death, head of the East Wing, Tarzan appears and smashes an inmate's head with his bare hands. The inmate's head has been replaced with a dummy prop head in the shot where his head gets smashed. (00:39:19)
