Factual error: After Red receives the package containing the tooth, it cuts to a city image that reads "Warsaw, Poland" but the city shown in this image is not Warsaw, it is actually Budapest, the capital of Hungary. This specific image shows the Széchenyi Chain Bridge (Széchenyi Lánchíd) and the Buda Castle (Budavári Palota), in Budapest. (00:06:50)

Visible crew/equipment: When Flores discovers her cell phone is missing from the hole in the wall she goes nuts, slams the stall door and kicks it in. After she is taken away there's a closeup of the stall and we can see the metal spring that was duct-taped to the bottom of the wall inside.
Continuity mistake: When Adrian stamps "denied" onto Boyle's insurance claim form, there's blank spots where there wasn't enough ink on the stamp (which would happen because he had the stamp in his pocket for a while). But then when he says "denied" and holds up the form, "denied" is all filled in fully. Also, the red border is lighter and the location has shifted down.
Visible crew/equipment: A red T-mark can be seen on the floor when Linda takes Rattray to Medical, telling Linda's actress where to stand in the scene.
Factual error: In the scene, Floki walks towards a small waterfall today known as Kvernufoss. Floki is supposed to be the only inhabitant if not the entire Iceland, at least the region, as we are shown no sign of civilisation whatsoever, and as historical record tell us about Iceland in the early 800s (as the events of Vikings seem to take place). But a well beaten footpath is visible, leading to the waterfall at the end of the canyon. That path is an oft-travelled path tourists follow nowadays to reach Kvernufoss, a well-known site in the area. A path like that in Floki's time would simply not have existed (there is nothing in that canyon but that waterfall, no reason to go there if not for sightseeing) and represents a (tiny) intrusion of our modern world in a show set in the middle age Iceland. Continuity mistake: When they're on the phone with Adam, between shots the phone switches hands.
Factual error: Throughout the show bottles of liquor have sprout dispensers attached to them. Sprout dispensers were not invented until the 1960s.
Something Ricked This Way Comes - S1-E9
Continuity mistake: When Rick, Jerry and Beth are eating pancakes, Jerry has his fork in his, but when Morty walks into the room and Jerry mentions science fairs are a father/son thing, Jerry's fork is suddenly on the table beside the knife. A further two shots after, the fork is suddenly back in the pancakes.
Chapter 24 - S2-E11
Other mistake: In the end credits, "Frank Underwood" is misspelled as "Frank Undwerwood."
Blue on Blue - S1-E5
Continuity mistake: At the end of Outbreak, Big Jim finds Angie down in the bunker. Water is flooded everywhere (knee-deep), and Jim has on an orange buttoned long sleeve shirt. When this episode starts, at 2:15 the water is practically gone, and Big Jim is in a blue short-sleeved polo shirt. (00:02:15 - 00:40:15)
Suggested correction: Jim shuts the door when he discovers Angie, then he comes back the following day and that's why his shirt is different.
Other mistake: When Norman and Madeleine are talking outside the Motel about Sam, the left side of the screen appears to be split down the middle after Norman says "Because you know it's safe to tell me", causing a weird effect on the chair that Madeleine is sitting in.
Continuity mistake: When Agent Ward goes onto the aircraft to find Agent Fitz and Agent Simmons having an argument, Simmons put a cup of coffee on the desk. It vanishes from the desk a few seconds after she does this. (00:15:40)
