Continuity mistake: In the scene where Dakota is having sex with Aubrey's boyfriend, it specifically shows that her bra is off. Once they finish, and she is getting off of him, she is covering her chest with the sheet. When she does this, her side isn't covered and you can that she is now wearing a black bra.
Continuity mistake: When McClane is driving the police down the ramp to the tunnel so he can elude the helicopter, the car's windshield doesn't have any bullet holes or cracks in it, even though it did just a few seconds earlier. (00:37:55)
Continuity mistake: In the scene Where Mike Eslin's hotel room has frozen over and he is laying on the floor talking into his recorder he notices his computer turns on and his wife is talking to him. As Mike tries to get up you see his head touch the alcohol bottle, in the next shot that is further away from Mike you see the bottle is much further away from his head then it should be.
Continuity mistake: In the scene when Hannibal returns from Lecter Castle after the murder, his aunt is waiting for him in his home. In the close up of the aunt Hannibal's collar is up, in Hannibal's close up his collar is down. (01:05:15)

Plot hole: In the scene where 47 swings out of the hotel window, we see the booby trap starting to explode as he's still in the room. Then in the shot from outside, the explosion extends out the window - so how on Earth did he outrun it and get out completely unscathed?
Suggested correction: He escaped before the debris from the explosion reached him. If you watch in slow motion, you can see that he was ahead of the debris whilst in the room, then below it when out of the window. And even if he had been hit by some, it wouldn't have been a plot hole, unless he was killed or seriously injured.
Continuity mistake: When Rigg is watching the tapes of Jill's interview he picks up a guide to civil engineering book. Camera does an instant cut and now he's got a police file in his hand. (00:15:30)
Revealing mistake: When Stevie is looking around in the desert before being shot, you can see the outline of his facial makeup around his eyes. (00:17:50)
Continuity mistake: During the banquet, Semyon motions to the waiter that offers to pour him champagne, to serve the guest to his right instead. The bottle was showing the label to the camera, but in the new shot it made a 180° turn, the label faces the waiter instead. No, it could not because the shot was meant to be continuous. After Memphis spits out the water, we see the guy holding a half filled bottle start pouring water into Memphis' mouth. When the camera angle changes (i.e. continuous shot and not a jump cut), we still see the guy holding the bottle up to Memphis' mouth. It's at that point the bottle has changed and is now almost full.
