Visible crew/equipment: When Ethan is trying to get into the building for his interview you can see the lapel mic on his back. He leaves the building, runs back in through another door, a guard flips him and you see it on his back.
Visible crew/equipment: In the scene of the funeral for Arnold's wife and son, we hear the sounds of "Amazing Grace" being played on the bagpipes. There is a bagpiper in the distance, as the hymn goes on the piper can be seen blowing up his pipes and setting them under his arm without missing a note, which is physically impossible. When the camera pans to the gravesite, there is a second bagpiper in the foreground. He does the same exact thing, he blows up his pipes and sets them under his arm mid-tune with out missing a beat. When playing the bagpipes, you must fill the bag up with a certain amount of air to make the drones (the three pipes that come out of the bag and rest on the piper's shoulder) sound. You must then blow more air into the pipes to sound the chanter and play a tune. You cannot do what the bagpipers in this movie were doing and still play the tune.

Visible crew/equipment: In the old man scene at the end of the movie after the credits roll, Ryan Dunn was supposedly launched into the air by an explosion. If you look closely after he hits the building you can see the cable that is lifting and holding him up.
Visible crew/equipment: The mistake can be seen during the montage where the whole family is sick. Campbell Scott is attending to his sick wife, Hope Davis. He comes to the bedside to bring her a drink. As he leans down to sit on the bed and the camera follows him, a boom microphone can be seen edging into the top of the screen on the right side. It is quickly pulled back. This was witnessed during a DVD viewing.
Visible crew/equipment: When Clint Eastwood dreams he is being shot by the masked man in the party store, you will notice obvious padding or vest under his shirt as he is being shot. This is padding for the blood charge(s) used for special effects in the film.
Visible crew/equipment: When Dr. Lee is telling Devon not to look down you can see the white reflector in his sunglasses.

Visible crew/equipment: When the wounded Aragorn gets up on Brego the next shot shows him ride towards the sun, and his shadow behind him. The same thing goes for the camera crew's shadow. The shadow of the crew is seen at the very beginning of this shot. And there are no rocks beside the camera because the camera moves 180 degrees to show open terrain.
Visible crew/equipment: When Toba is skating simple-mindedly on one side of the track (before he gets hit), you can see the Rollerball fans are just cardboard cut-outs of people.
Visible crew/equipment: During the first fight scene at the beginning, the snow looks extremely fake. One shot zooms in on a man's foot. As he lifts it up the snow is lifted as well, like a sheet of cotton.
Visible crew/equipment: In the "Roxie" number, just after Roxie sings "that means somebody everyone knows" she goes down a line of mirrors and as she does you can see three crew members with what looks to be a camera in front of the middle one. Their figures are somewhat dark, so it's a bit hard to see, but it's there.

In the upper right corner of the frame, there are someone's feet/black shoes and edge of a garment dangling from above. If they don't belong to a crew member, perhaps they belong to Ellie, her part in the scene having been cut out.
