The Secret Box / Band Geeks - S2-E14
Audio problem: At the end of "Band Geeks" when everyone is playing "Sweet Victory" at one point Mrs. Puff is shown playing the guitar. When this happens, you can see the others in the background not doing anything, even though their instruments are heard.
Audio problem: At the beginning of the episode when Cordelia leaves with Pierce, she says, "Don't mind him". But her lips don't match the words.
Audio problem: When Peter introduces the pope to his friends at work, and he is saying "Remember when we got that stripper for Charlie and it turned out to be his son? I think this is gonna top it" or something along those lines), Peter's mouth very obviously does not sync up with the words being said.
Shibboleth - S2-E8
Audio problem: As Charlie leaves the Oval Office, he pulls the door behind him. When it stops moving, the audio suggests it has been closed fully ("ka-chunk"), yet it's obvious it has not been closed as we can still see the outer office's curtains at the right of the screen, confirmed further when C.J. steps forward and the camera pans right to show the entire door frame area, and the door open quite a bit. The Honking - S3-E1
Audio problem: After they have welded Bender to the wall Leela and Fry leaves. When they go outside the door is closing. It cuts and we hear the door close but we can see the door is already closed. (00:11:45)
Audio problem: In the first long shots of Visiting Day playing at the club, the lead vocal is heard, but the singer is standing well back from the mic and does not appear to be singing.
Inheritance - S3-E8
Audio problem: Benson and Stabler are searching Darrel Guan's room. When Stabler picks up the bloodstained pants, Benson 'says', "blood" but her mouth doesn't move. (00:28:05)
Audio problem: After Nasedo reveals himself in the morgue, there is a shot on Michael's face. There is no sound, but Nasedo can be seen in the foreground either talking animatedly or having an extremely localized seizure. (00:23:55)

Audio problem: During the Denton's song when they are bringing Benjamin back to the bed, Auntie Val is singing in the hallway but her mouth isn't moving.
Guilmon Comes Alive - Part 1 - S3-E1
Audio problem: When Jeri starts miming with her sock puppet, it is sometimes out of sync with her voice. It is most noticeable in the part where Takato has detention.
