Be Careful What You Witch For - S2-E22
Plot hole: When teenage Prue has run away with the demon, and Phoebe and Piper find her in the park, teenage Prue is 'killed'. Her sisters drive her home to get Leo to heal her, but shouldn't they have just been able to call to Leo to orb to them when Prue was first hurt, and heal her in the park, and save driving back to the Manor, risking Prue dying?
Suggested correction: I see orbing to the park but he didn't heal her, she was already dead and he can't heal the dead, that's why the genie brought her back - that's what Phoebe wished for.
But if they would have called for him right away - he would have been able to heal her before she died. However at this point in time I don't think they knew how to call for him.
Continuity mistake: When Kitty comes downstairs to give the gang cookies, she tells Eric that she picked out an outfit for him to wear on career day. He then turns and mouths the words "Help me." In the next shot you can see Jackie sitting on Kelso's lap when in the previous episode, she broke up with him. In the next shot she is standing behind him. (00:01:00)
Revealing mistake: When Carrie phones Doug at IPS, Doug goes to press the button on the phone for the correct line, but doesn't press any and continues to talk to Carrie. (00:07:40)
Other mistake: During the Pokérap, when the Poliwrath name is said, the Pokémon shown is actually Poliwag. Also, when the Graveler name is said, the Pokémon shown is actually Geodude.
Audio problem: When Carrie is meeting with Sarah Michelle Gellar and she is told to guess which star is interested in being involved with her project, SMG mouths 'Warren Beatty' and not Matthew MConaughey as is heard.
Revealing mistake: In the episode where Denise tries on her wedding dress Jim leaves the room to go and wash up. As he opens the door to leave the living room if you look down towards the front door you can see Dave stood by the door frame waiting for the next scene.
Continuity mistake: Takumi and Shingo both have their right hands duct taped to the steering wheel, as per the rules of the Gum Tape Deathmatch. During one shot of the race, Tak's hand is no longer taped and resting casually on top of the steering wheel as opposed to the right side of it.
Continuity mistake: When Becker offers Dr. Elizabeth a beer from his fridge there is an orange inside, when it cuts back to Becker the orange is gone.

Plot hole: The children decided that they hated the Powerpuff girls in one day. Then how did they have supervillain costumes and a van? The mom said they had lots of time to prepare.

Factual error: There is no megatron at Wrigley Field.
Two Guys, a Girl and a Vacation - S2-E2
Continuity mistake: In Two Guys, a Girl and a Vacation, Pete is drinking brandy with a man called Peter Faber. Peter Faber's glass is only a quarter filled, but a few shots later the glass is almost half full. It is impossible that he could have poured more into the glass, and the waitress couldn't have walked all the way down to the table in the few seconds it lasted for the angle to change.
Revealing mistake: When Rachel and Adam first meet, their cars crash in the car park. In the shot where they both get out of their cars, you can see a dry patch of ground under Rachel's car, but the rest of the area is wet. Why would that space be dry, if Rachel just crashed and it isn't raining heavily at the time?
Plot hole: During the episode "Superstition", Onizuka thinks he has cancer because the magnetic pain-pads he was wearing created strange blobs in the X-Ray he has taken. However, given the fact that the X-Ray was so strange, doctors would have given him a complete physical, and noticed the pads much sooner. Here, they literally just assume he has tumors, and fail to ever take notice of the pads. Simply impossible, given the gravity of the situation. (Obviously, this is to pay off at the very end of the episode.)
Sammo Blammo - S2-E1
Continuity mistake: In the scene at the bank, he has a bomb attached to him and an ex-bad guy handcuffed next to him, the handcuff is roughly a foot long, making the distance short between him and the guy next to him. Sammo fights a gang of thugs in the bank and does a swing round kick and his handcuff is detached or widens by 3-4 ft. The guy on the floor is rolling around to make it look like he is attached to Sammo, but is clearly not.
Visible crew/equipment: In the opening sequence, when Karl opens the door to his apartment, you can see the top of the set wall and the stage lights hanging above, in the corner of his living room.
Continuity mistake: In the episode "Fool's Gold" Debbie puts the fool's gold on top of the pail on her sculpture. The shot changes and it's gone, the next shot it's back.
