Audio problem: When Mrs. Moore calls for her dog, Pookie, the sound does not match her lips moving.
Audio problem: 1897: Quentin challenges Barnabas by pulling a decorative sword from a wall display and pointing it at his throat. Unfortunately, the display wasn't all that well attached to the set wall. Though the camera pans away, you can hear a loud clunk as the piece falls, followed by the loud crash of shattering glass as it strikes and breaks a table lamp. You can also see the actors struggling very hard to keep straight faces as they continue with their lines.

Audio problem: When Sarah goes to touch Evelyn's shoulder, you hear Sam's voice telling her to stop, except his mouth isn't moving at the time the phrase is being uttered. (00:23:50)
Audio problem: When Jerry uses the elevator, the voice of the computer reads out the floors as "twenty-third floor, fourth floor, fifth floor" etc, missing out the "twenty-" prefix until the 29th floor. The DVD subtitles list these numbers with their prefix, so this was not intentional. (00:21:10)
Audio problem: In episode 1x08, "Swimming With Boyd", when Lucas is talking to Julie on the street, the audio looping as he says "Listen to me" is quite noticeable - his lips don't even come close to matching what we hear.
When the Dead Come Knocking - S3-E7
Audio problem: When Rick and his 'strike force' hole up in the cabin surrounded by walkers. They arouse the cabin's owner who brandishes a double-barrel, breech breaking shotgun. When characters brandish pump action shotguns, often they pump a round for effect and the sound effect is characteristic. The pump action sound effect is erroneously applied to the breech breaking double barrel shotgun.
Audio problem: The keys we see being depressed on the player piano never bear any resemblance to the actual notes we hear. (00:13:00)
Chapter Four: The Moveable Feast - S1-E4
Audio problem: When Father Keane is speaking with Mother Bernadette outside, when Mother Bernadette says "The gift doesn't reside in you or me", and "Where you coerce and compel, we use compassion, forgiveness, and patience", her lips do not match what she is saying.
Audio problem: The zombie chasing the man gets tangled on a wire fence and gets shot in the head. When the bullet impacts his skull he makes a grunting pain noise just like everybody else who gets shots in movies makes the same grunt. First of all zombies don't feel, so how can he express pain, and secondly when people get shot and die they don't grunt, they drop dead. (00:09:30 - 00:10:00)
Audio problem: When Roman is addressing Bill and Eric, as he says, "To meet under these circumstances", his lips don't move until he says the word "circumstances". (00:46:05)
Lust for Royston Vasey - S2-E2
Audio problem: During the Denton's song when they are bringing Benjamin back to the bed, Auntie Val is singing in the hallway but her mouth isn't moving.
Audio problem: When Brian escapes his room and the headmaster with security is checking the rooms, if you look very carefully while the camera is going to Brian's room, you can see the headmaster hitting the door with his hand out, but the sound sounds like it would if he were hitting it with his fist.
Audio problem: During Kenzie's speech to Dyson at the gym, at one point when she asks "All of that just to give up now?" Dyson answers something, but there's no audio. (00:18:15)
Audio problem: After Deputy Lin drops the line "we'll get the results back fast," Sheriff Alex Romero responds with "I'm not sure how much nail clippings are gonna tell us." As he sits down, his mouth movement matches the audio, but when the angle changes, his mouth isn't moving at all. (00:09:55)
The Addams Family Meets the Undercover Man - S1-E16
Audio problem: Ted Cassidy (Lurch) was an accomplished organist. However, when he "played" the Harpsichord, he was only miming the actions. When Lurch is playing while Gomez and Morticia are fencing the music doesn't match the movements of his hands.
Audio problem: When Ilonka is speaking with her father about going to Brightcliff Hospice, in a shot where the camera is behind her, she says "Lots of foster kids. I mean, kids on their own" and the line appears to sync with her mouth perfectly. But immediately after finishing that line, she starts saying "It's a place for teenagers to transition on their own terms", and her mouth doesn't appear to sync up with the audio at all. (00:12:50)
Audio problem: After talking with Kieran near the start of the episode, Emma (off-screen) gets into her car, shuts the door, turns on the engine and starts driving away in 3 seconds flat.
Audio problem: In the house as the walker is shot nearly point blank, the sound of the shotgun goes off before the flash.
Audio problem: When Olivia calls Astrid to tell her it wasn't Walter's research, we cut to Astrid who says, "Good news", but her lips do not move. (00:33:57)
Audio problem: When Decker and the professor walk across the college campus, there's a badly-looped bit of dialogue that causes the professor's line to continue after his lips have stopped moving. (00:05:05)
