Audio problem: At the end of the episode, Buck comes home and crashes on his bed. He sees his girlfriend reporting on TV. Just before the shot cuts, the reporter mouths a word but nothing comes out. The shot changes to others watching the report, and she continues talking.

Audio problem: When Donatello tries to stop April from leaving the sewers after she accuses them of committing the robberies, he is speaking in Raphael's voice. (00:10:35)

Prey of the Black Panther - S2-E7
Audio problem: When Black Panther is saying "The Black Panther's ready to stalk again," he doesn't press his lips together when saying "Panther" in order to produce the "P" sound. (00:21:12)

Audio problem: When Anakin says, "What? Uh-oh," his mouth does not move. He just looks shocked that his lightsaber could not cut The Zillo Beast.

Audio problem: To satisfy his weird mother fetish, Hyoga dives with a rose in his mouth, but that does not prevent him from shouting his battle cry. (00:07:00)

Audio problem: 10k pulls a knife to stab a zombie through the fence at the nuclear plant but they dub in the sound effect for a gun when he stabs the zombie.

Audio problem: The zombie chasing the man gets tangled on a wire fence and gets shot in the head. When the bullet impacts his skull he makes a grunting pain noise just like everybody else who gets shots in movies makes the same grunt. First of all zombies don't feel, so how can he express pain, and secondly when people get shot and die they don't grunt, they drop dead. (00:09:30 - 00:10:00)

The Strength of the Wolf - S3-E4
Audio problem: One of the team members shoots a sentry on board a ship off a ladder attached to the deck hanging off the side of the ships hull. He shoots an unsurpassed Glock and the shot sounds suppressed. A muzzle flash can be seen too, another indicator it's unsuppressed. Their element of surprise would have been totally eliminated had this really happened. They've used suppressors before on their sidearms - doesn't make sense why they wouldn't now. (00:31:00)

Audio problem: When Talbot comes upon the Hulk in his helicopter, the helicopter doesn't make a sound until it appears on screen.

Audio problem: During the final run to the Boar's Nest, Luke is driving General Lee. There is one final jump, and you hear Bo's rebel yell, but Bo is at the Boar's Nest.

Audio problem: Coulson is talking to a cowboy who sold a piece of construction equipment to some bad people. The cowboy pulls a breach-loader shotgun. Ward grabs the weapon and pulls the cowboy off his horse. Next the distinct sound of racking a shell into the breach of a magazine-fed shotgun can be heard, but the breach-loader is seen.

The Phoenix Saga (5): Child of Light - S3-E7
Audio problem: Storm's mouth is completely closed when she says "The immediate threat to New York City has been averted." (00:12:10)

The Krakken - S1-E3
Audio problem: When the kraken roars, when it destroys a ship, it sounds like a TIE fighter in Star Wars.

Audio problem: When Oliver is fighting the guys in the hallway, he shoots the second to last guy. But when he starts to draw an instant beforehand, you can see that his right hand, with which he holds the arrow, is nowhere near the quiver when the sound of the drawn arrow begins.

Audio problem: When Carter is searching Jean Jackson's house she asks him if he has a warrant - although the camera isn't on her you can see her reflection in the mirror and you can see that her mouth doesn't open when she is asking Carter this.
