Continuity mistake: On the train, the man who approaches Reese wears a silver necklace, which jumps in and out of his top throughout the scene. (00:01:00)
Continuity mistake: A man tries to take Reese's bottle, only to be startled when the seemingly asleep Reese grabs his hand. Throughout the scene, Reese's scarf is either in his hand or not. (00:01:05)
Continuity mistake: While Reese and the witness are hiding in an apartment, the door is opened by the student to dismiss the bad guys with the chain lock on the door. The door is open about 18 inches and yet when the student closes the door the chain lock is normally sized about 4 inches and is still attached.
Visible crew/equipment: There is lighting equipment reflected on Carter's sunglasses when she confronts the FBI agent who is following her. (00:26:45)
Root Cause - S1-E13
Factual error: Finch's trojan tracker displays "accessing network on IP 623.173.93 .225.9." Not only is 623 an impossible number for an IP address to have, as they only go up to 255, it's also made up of 5 parts, instead of the 4 which IP addresses are limited to.
Visible crew/equipment: When Reese and Saunders are nearly crashed into at the construction site, as Reese is exiting the vehicle you can see the stunt driver is the one sitting in the driver's seat instead of Saunders. (00:18:45)
Continuity mistake: Mr. Reese and the stockbroker are hiding out with the street people. In the portion of the show entitled "Kill Squad," at about the 2:02 mark, Mr. Reese is building a fire in a steel drum. The paper and wood are nearly overflowing as he continues to prepare the fire. They cut to Mr. Reese spraying lighter fluid onto the pile of wood and paper. However when Mr. Reese throws in the match to ignite the fire, all the debris are way down in the steel drum. As the flames come upward there are no signs of the wood or paper that had been obvious before.
Continuity mistake: When Reese is following the Drakes into their office building and speaking to Finch via the earpiece, it is dark and raining; but when the shots change to Finch driving during the same conversation, it is not raining and is much brighter outside.
Continuity mistake: Reese orders the cab driver to smash through a car blocking their escape which causes considerable damage to the front driver's side of the car. This damage is shown in one of those computerized captured pictures this show uses throughout each episode. Then in the next scene, when the cabbie is standing alongside his now parked cab, the front driver's side is now perfect, but when he walks around the car to talk to Reese, the passenger side is damaged. The passenger side had been untouched in the crash.
One Percent - S2-E14
Continuity mistake: Immediately before Logan Pierce, who Reese is protecting, jumps into his fancy sports car to pull away from valet parking, the street is packed with traffic, has multiple lanes, a divider. Yet, in the next scene he is pulling out and accelerating the car in an empty single lane street with parked cars and trucks.
In Extremis - S2-E20
Continuity mistake: In episode S01E01, it is established Fusco is a detective in the Vice Squad, and later Finch arranges a transfer to Homicide to keep an eye on Carter soon after he accepts the offer for redemption. In this episode during a flashback Fusco is a homicide cop. (00:18:25)
Lady Killer - S3-E3
Continuity mistake: Root shoots the hitman, walks up to him and slides his gun a couple of feet away. A wide shot shows the gun where it should be. After she says "you're the boss" and starts walking away, we see exactly the same angle, the gun has vanished, and some random black bits have appeared.
Lady Killer - S3-E3
Factual error: They show a flight reservation for the young boy's trip to London from LaGuardia to Heathrow. LaGuardia is for domestic flights only.
Continuity mistake: The blood smear on the photograph pointing out the head of HR changes position between shots.
Continuity mistake: Harold, Shaw and Harold's friend escape from their captors early in the episode. They smash the front driver's side window of a parked car, which they hot wire to drive to the bank where the tapes are stored. When they arrive at the bank, the window is in perfect condition.
Panopticon - S4-E1
Factual error: John fires a grenade launcher at a street gang hangout bar front window. Aside from the fact that such a round would hardly have been slowed enough to just roll down the bar, the round is shown complete with cartridge case, which would remain behind in the launcher/gun - it doesn't fly with the round.
Factual error: The bounty hunter, Frankie's, car has a Florida license plate on the front of the car. Florida only issues one license plate for vehicles, and it goes on the rear of the vehicle. (00:15:15)
The Day the World Went Away - S5-E10
Continuity mistake: As Finch and Root are being chased by an SUV equipped with a mini-gun, Root uses a.338 Lapua to shoot the SUV engine four or five times. As Root descends back into the car the rifle in question is plainly a bolt action precision rifle. (00:28:55)
Factual error: Samaritan launches a cruise missile at the rooftop of 1133 Avenue of the Americas, where a satellite dish is located. A real US Navy Raytheon SM-6 missile, fired from a surface ship, would shoot down the cruise missile shortly after it was launched and long before it hit NYC.
