I Ain't a Judas - S3-E11

Corrected entry: When the governor first puts his eye patch on it is on his left eye. When he turns around his eye patch is now on the right eye and stays this way throughout the rest of the season.

Correction: When we see the Governor put his eye patch on it is his reflection, we see which is why it is on the left.

Made to Suffer - S3-E8

Corrected entry: When Tyreese and his group are in the woods before they find the prison, Ben's mother gets bitten on the arm, but she starts limping and needs to be held up by her husband.

Correction: Ben's mum begins to limp because she gets caught in something as they're running away from the walkers.

Killer Within - S3-E4

Corrected entry: When the Governor is teeing off on top of the walled barricade, we can see the tree line in the background is far away at the edge of The Other Side of Woodbury. After Merle has a chat with the Governor, we now see the trees are magically a lot closer.

Correction: The trees were the same, they are different distances depending on the angle and eye coordination.

Killer Within - S3-E4

Corrected entry: Carol's head wrap is well intact as she and T-Dog find escape down a hallway during the alarm system chaos. Soon they run into another pack of Walkers, and he sacrifices himself in order to save her life. As she runs past him, and toward another escape door, her head wrap is still intact as she opens the door and runs off. Later, when Rick and Daryl happen upon that same hallway, and T-Dog's remains, Daryl glances downward, however, and picks up the same head wrap, now covered in blood, alongside her bloodied knife.

Correction: We do not see what Carol does after she leaves. It is possible that she returned and left her head-wrap. Also, her knife was found later on in the episode.

Clear - S3-E12

Corrected entry: In the corner cafe, Carl grabs his family photo and then he and Michonne attempt to sneak back out to safety, but get run in by Walkers. As she downs a few she and Carl run the other way. However during the escape, he accidentally drops the photo in the middle of the walkers as they reach the front doors to safety. Outside, he insists on going back for his family photo but Michonne tells him to stay put... She will get it. Moments later she returns holding the family photo. Since the photo was dropped near the front doors, which is occupied by the Walkers, this would have been impossible for Michonne to have accomplished.

Correction: This is not a plot hole. We do not see what Michonne does to retrieve the photo. She most likely killed a number of walkers to get through - we know she can handle a large group of walkers on her own so she most likely fought her way through.

Consumed - S5-E6

Continuity mistake: When Daryl is driving the hospital van, it shows that he is going to land upside down, on the vehicle's roof, when he drives off the parking garage. In the next scene, the vehicle lands on its wheels, as if it has done a complete front flip. (00:30:00)

Question: Has there been any explanation (in-show or by show creators) about the inconsistency of what attracts Walkers? They've shown that people who cover themselves in Walker guts aren't detected or attacked. When it rained, the rain washed off the guts and then the Walkers do try to attack. But if people hide under cars or don't make noises, the Walkers don't detect them. But it's also been shown that people under a pile of "dead" walkers are still attacked. So what causes a Walker to attack?

Answer: Walkers are attracted to noise, they can tell who's dead and who isn't dead, in S1 when Rick and Glenn cover themselves in walkers' guts they're able to get far by walking through the herd of walkers because they smell like they're dead, but as soon as the guts get washed off by the rain the walkers immediately attack them, if you draw attention to yourself they will attack you which we've seen that many times throughout the series that it's not always successful. Whether you're covered in guts hiding under dead corpses or under a car.

Answer: Walkers use sight and sound to find their prey, with sound being their first choice. When the characters cover themselves in guts, they aren't attacking because they can't "see" them and they're not making any noise. The same goes for when they're hiding under cars - walkers can't hear or see them. They'll go for anything that makes a noise, regardless of what it is. It's also very likely that walkers don't have the ability to smell, since they can't find people hiding under cars. Nor can they smell the Whispers who walk amongst them.
