Beyond Good and Evil: Part 4 - S4-E11
Continuity mistake: The same time portal with the image of the neanderthal by the fire is erased by Apocalypse twice. (00:12:45)
Beyond Good and Evil: Part 4 - S4-E11
Audio problem: After Bender reverts to his true form of Immortus, he laughs before disappearing. The laugh is oddly inaudible on the DVD. (00:18:50)
Beyond Good and Evil: Part 4 - S4-E11
Continuity mistake: The end of the episode shows Xavier standing with no assistance. The only time in the series that the use of his legs were restored were a side effect of him losing his powers. He has his powers in this scene, so there's not really any reason for him to suddenly have the use of his legs back. (00:19:15)
Have Yourself a Morlock Little Christmas - S4-E12
Other mistake: Rogue states that this is Jubilee's first Christmas as an X-Man. However, Jubilee was already a member of the X-Men when Senator Kelly declared his candidacy for President in season 1, and the season 2 premiere episode showed him being sworn-in as President. Election Day is in November and the President is sworn-in in January, so Jubilee should have already had at least one Christmas as a member of the X-Men. (00:02:00)

Have Yourself a Morlock Little Christmas - S4-E12
Continuity mistake: The boxes and ribbons in the stack of gifts that Jubilee is carrying change color. (00:05:20)
Have Yourself a Morlock Little Christmas - S4-E12
Other mistake: When Ape and Annalee walk to the back doors of the ambulance, the window on the right rolls down. Ignoring the fact that those windows don't actually roll down, neither Ape nor Annalee do anything to make it roll down. (00:06:55)

Have Yourself a Morlock Little Christmas - S4-E12
Continuity mistake: Jubilee's presents suddenly become smaller when she, Storm and Wolverine depart for the Morlock tunnels. (00:08:45)

Have Yourself a Morlock Little Christmas - S4-E12
Continuity mistake: When Leech is first shown in his sick bed, he is shirtless and under the sheets. When he begins receiving a blood transfusion from Wolverine, he is fully clothed and also wearing a beanie. Given the weak and near-death state he was in, it's extremely unlikely the other Morlocks would bother dressing him. (00:09:20 - 00:14:15)

Have Yourself a Morlock Little Christmas - S4-E12
Continuity mistake: Callisto's eye-patch changes sides when she is berating Storm. (00:09:30)
Have Yourself a Morlock Little Christmas - S4-E12
Other mistake: Jubilee should not have been able to see Mariana without looking around the corner. (00:10:25)
Have Yourself a Morlock Little Christmas - S4-E12
Continuity mistake: When Jubilee is holding Mariana, her right arm goes from cradling Marian's head to being under her bottom. Jubilee also goes from wearing yellow gloves to being bare-handed. (00:11:40)

Have Yourself a Morlock Little Christmas - S4-E12
Continuity mistake: The brown patch on Ape's left leg disappears when he gets up to assist in Wolverine's blood transfusion for Leech. (00:13:15)
Have Yourself a Morlock Little Christmas - S4-E12
Continuity mistake: Rogue's headband is flesh-colored when Beast says "It would appear that my face is not the world's most reassuring." (00:16:30)

Have Yourself a Morlock Little Christmas - S4-E12
Other mistake: When Gambit is tasting the food in the crock pot, his left arm has been drawn backwards.
The Lotus and the Steel - S4-E13
Other mistake: When Wolverine is telling Xavier about his road rage incident, his jeep is missing its windshield. (00:03:20)
The Lotus and the Steel - S4-E13
Factual error: When Wolverine tears off the man's car door, he does so by putting his hands inside the spaces between the car door and the car's body. Those spaces are way too narrow for a person to fit their fingers inside. (00:03:25)
The Lotus and the Steel - S4-E13
Deliberate mistake: Every native of the Japanese village that Wolverine visits is conveniently fluent in English, and they even speak English to each other when no other people from English-speaking countries are around. (00:04:45 - 00:20:45)
The Lotus and the Steel - S4-E13
Continuity mistake: The village elder Yoshida-san suddenly has sideburns and a goatee when Soichi is mocking Wolverine. He is clean-shaven for all other parts of the episode. (00:07:25)
The Lotus and the Steel - S4-E13
Revealing mistake: The footage of Jubilee flying off in the mini-jet is recycled footage of her flying Colossus back to Russia from the season 4 episode "Red Dawn." You can even make out Colossus' silhouette in the window over the passenger seat in one shot, even though Jubilee is supposed to be by herself. (00:07:35)
The Lotus and the Steel - S4-E13
Other mistake: The camera is out of focus when the Silver Samurai threatens the villagers. (00:09:35)
Answer: Chris Potter, the original Gambit voice actor, did indeed quit the role in the 4th season. His last episode was The Phalanx Covenant Part 1 so it seems reasonable that Fox would limit the use of the character even though it was recast. It doesn't appear that Potter left the role due to animosity, he stated in an interview that he wished to play Gambit in the first live action X-Men film.
