Visible crew/equipment: After Derek tells Cristina that her patient is paralyzed, when it cuts to the ICU as Cristina talks to the man's wife, the pink T-mark becomes visible on the floor at the bottom of the screen, near the right side. (00:02:05)
Other mistake: When Derek has finished showing Callie and Arizona the picture of Cristina getting her nomination, he leaves to take the kids into what we are supposed to believe is daycare...however the sign on the door reveals it is a dispensary. (00:02:30)

Other mistake: When Meredith is talking to Kenzie about the conjoined twins, she is wearing a lab coat that says Seattle Grace Mercy West. The hospital is named Grey Sloan Memorial at this point. (00:20:00 - 00:21:00)

Continuity mistake: In the Yang/Grey scene, Yang is scrubbing and pulls back her mask, seconds later the mask has disappeared. Also her arm has soap on it when Meredith pulls it out, but then suddenly the soap is gone.
Could We Start Again, Please? - S11-E7
Visible crew/equipment: In the ER, when they move Hattie off Norris he grabs hold of her hand, and two shots later in the closeup as Norris lets go of Hattie's hand, the white T-mark is visible on the floor. (00:04:55)
Crazy Love - S11-E19
Visible crew/equipment: You can see the camera recording in the glass when panning over Arizona.
Other mistake: The exact same shot of Derek's radio tuner is used twice-once when he calls Amelia and again when he gets in the car after helping at the accident. Both times, the clock says 7:45PM, but Alana (the AP class taking, cello playing student) says that she's supposed to be at school, implying it's sometime between 8AM-ish and 3PM-ish. Also, the sun sets around 7:30PM in Seattle on March 26, but it is obviously day time for the duration of this scene so there's no way it could be 7:45PM in either of the shots in which the clock is shown. Continuity mistake: Arizona is wearing fitting black slacks and black heels and has two legs (not a prosthetic on the left). (00:37:45)
Factual error: At the end they land at Sioux Falls Airport or "Joe Foss Field" but the shuttle says "Joe Boss Field".
Visible crew/equipment: At the start, when Jackson and Maggie react to the fire alarm, just as Maggie says, "All right fine. It's the guy. The bad, escaped rapist guy," the white T-mark is visible on the floor, before she stands up. (00:00:35)
Continuity mistake: When the episode ends and Jo and Alex are throwing balls at the wall, the position of the bag below changes between shots.
A Hard Pill to Swallow - S16-E11
Visible crew/equipment: After Hayes asks Meredith if she knows Dr. Yang she avoids an answer and walks away, then when it cuts to Bailey as she walks toward Koracick, three tape marks (two red, one blue) are visible on the floor near Koracick. (00:36:30)
Visible crew/equipment: When Teddy and Amelia walk over to Koracick he gives Amelia the boy's tumor diagnosis, and as Amelia walks into the patient's room the red T-mark is visible on the floor, at the bottom, left side of the screen. (00:15:10)

Visible crew/equipment: When Hunt and Hayes decide to get a drink at Joe's after work it then cuts to Schmitt standing at the doorway, and the blue T-mark is visible on the floor just as Kim joins Schmitt. (00:39:20)
