Audio problem: When Gretchen kills the guy in the prison he lets out a very obvious fake scream similar to a Wilhelm scream. Also, he is being choked around the neck, how is he able to make the scream? Take note of when you see this scene in the next episode's review. The guy doesn't make the scream in that version.
Continuity mistake: The shadows and lighting change throughout the scene with T-bag giving the money to the prisoners. At one point he is in shadow, the next full sun.
Visible crew/equipment: Gretchen arrives at the pickup spot with the van with LJ inside of it. As she opens the back door the camera moves closer and the reflection of the crew in the glass is obvious. (00:08:30)

Visible crew/equipment: When Whistler talks to Sophia about Lincoln, the camera starts from a high angle. It moves down and to the right. During this time it casts a very obvious shadow on the fence.

Continuity mistake: The same aerial shot of the prison tower from the previous episode is shown again near the beginning of this episode. Take note of the vehicle driving to the right. Some time has passed between the previous episode and this one.

Other mistake: Sucre is supposedly passed out, however when Lincoln puts him on the couch his arm moves upward.

Continuity mistake: The way Gretchen holds her martini glass when first talking to Lincoln shifts during camera angle changes. (00:36:45)
Visible crew/equipment: When Sucre gets pulled over, the camera tracks along the driver side of the car. The camera operator is briefly visible. (00:25:25)
Revealing mistake: Michael and Lincoln climb over the dirt pile and down to the other side. The dirt area below them has been disturbed from previous takes.

Continuity mistake: Quinn falls into a well and you can see his cell phone sitting right next to him. A few minutes later he grabs it from his pocket as if it's been in there the whole time. (00:30:00 - 00:34:30)
Continuity mistake: When Lincoln Borrows is secretly talking to his son in prison using a cell phone, he is seen wearing an inner blue plaid shirt for most of the scenes before. As the conversion starts and focus switches between the two, the inner shirt is visible and not visible. It disappears and re-appears three times during the conversation. (00:20:00)

Revealing mistake: After T-Bag has been confronted with the death of Nieves and Nieves' body is shown on the floor, his stomach moves as if he is breathing. (00:32:30)
The Legend - S4-E10
Audio problem: When Mahone is speaking to Baker, just after Michael goes under the X-Ray, Baker says ' My goal is to build one city that entirely self sustains'. After this, Baker's words do not match the actor's mouth, from the shot behind him; 'Depending on no outside resources'. (00:19:45)
Continuity mistake: When Mahone is in Scofield's apartment the show flashes back to an overhead view of Scofield planning on the wall while Mahone is tracing over the holes in the wall from where Scofield was tacking papers up. The mistake is in the flooring, the laminate flooring is a much lighter flooring than when Scofield was there two months prior. (00:21:00)

Continuity mistake: Link pulls the clip out of his pocket, and the way he holds it changes between shots. (00:39:00)
