Continuity mistake: When Nancy goes into her guest room with Silas and Quinne in it, the things she are holding change position in between shots. (00:28:35)
Continuity mistake: When Nancy is in Doug's office, the items on his desk change position in between shots. (00:04:20)
Continuity mistake: When Nancy sees the video tape of her husband, the items on the table vanish in the next shot. (00:27:50)
Plot hole: If Andy really did enlist into the Army Reserves and "forgot about it" like he claimed a warrant for his arrest for desertion would have been issued, which the police would have seen when he was arrested for pot possession in the episode "Lude Awakening." After seeing the warrant, the police would have called the Army, and he would have been returned to military control to face desertion charges. He would not have been released and sent a letter a few weeks later simply telling him to report for duty or face military prison; the option to report for duty would have no longer been available when he was gone long enough to get put in deserter status.
Continuity mistake: When Nancy is speaking with Heylia in her kitchen, the red pot behind her on the stove disappears and then reappears.
Continuity mistake: When U-turn is going to investigate Sanjay's sneeze, he is pointing the gun over his wrist. In one shot, his right wrist is resting on his left wrist, in the next the barrel of the gun is resting on his wrist and it changes back and forth. (00:07:05)
Factual error: Nancy takes her son Silas to an 'underground' doctor used by gang members following an attack by a biker gang. After she tries to negotiate a lower price due to the lack of anesthetics, the doctor says that he can get a second opinion at a hospital and order a $10,000 MRI if she would rather pay for that. However, an X-Ray is the first, less-expensive method used by emergency rooms and trauma centers to confirm a fracture. An MRI is typically only used to evaluate skull/brain-related injuries, but is a rarer utilized, expensive second option to evaluate a fracture that generates an unclear X-ray.
Continuity mistake: When Roy Till and Ignacio are bound to the bed by Nancy, the black ties around Ignacio's left wrist keep changing places between shots.
Continuity mistake: When Silas is painting the wall green, some parts jump from being painted to unpainted between shots.
Other mistake: When Andy pulls his piece of meat out of the fondue pot it hasn't been cooked at all. The fondue obviously hasn't been turned on (in the next shot he sticks some meat in his mouth, which has been cooked and is smaller).
Continuity mistake: When Andy walks towards Gayle to give him the Claritin, he steps over the sign that Gayle was holding moments earlier, only for it to appear in Gayle's hand in the very next shot.
Visible crew/equipment: When Andy talks about naming the baby "Bernardo", a boom mic can be seen reflected in the glass of a cabinet to his right.
Continuity mistake: When Audra is talking to Andy outside of the clinic, everything about her cigarette changes between shots (position, smoke, etc).
Continuity mistake: When Shane is on the couch eating, his spoon goes from beside the bowl to inside the bowl when the shot changes.
Continuity mistake: When Nancy starts her maid job at the hotel she enters a room with a wig looking around at all the mess. When the shot changes to her going to the restroom lifting the toilet lid she has no wig on, then when the shot switches back she has her wig back on.
Revealing mistake: Right after Ignacio tells Andy and Silas to sit down with him at the diner, it's painfully obvious that the baby that Silas is carrying is a doll.
Visible crew/equipment: When Nancy gives the story about being aware of her people there's a flashback to when she's at the space exhibit with Stevie. If you look to the right of the screen when she's hugging him, you can see the shadow of the boom operator's microphone being held above their heads on the back curtain behind Stevie and Nancy. (00:27:20)
Continuity mistake: Doug has his son Josh kidnapped from his own home twice. The second time Josh answers the door wearing a dark long sleeve shirt, but when they cut out for a second and go back, Josh is bagged in the hallway wearing a short sleeved, lighter shirt. It's the same shirt he wore the first time.
Answer: It's not uncommon for TV shows to suddenly "age up" baby/toddler characters in order for them to have more narrative purpose. Other examples of shows I can think of that did this are "Step by Step," "Last Man Standing," "Boy Meets World," and possibly "Dexter."
