Continuity mistake: When Peter signs the consent form for his surgery, his head is raised off his pillow a few inches. The shot changes and he continues to sign - but now his head is lying on the pillow even though he didn't have time to move.
Continuity mistake: When Mrs. Novotny offers to get aspirin, Angie helps her up by Mrs. N.'s left hand - there's no blood on it, but the next shot shows her hand quite covered in dried blood. Mrs. N. has no blood on her clothes, despite being right next to her husband while he bled to death and Carol administered CPR.
Continuity mistake: Carol is working on James' injury and Duncan goes to answer the phone. James has his arm wrapped around the phone but the shot changes to focus on Robert - we can still see James but his arm is now at his waist, even though he had no time to change positions.
Continuity mistake: While Carol is trying to save Mr. Novotny's life, Mrs. Novotny's hair alternates from being loose and hanging over her cheeks to being neatly tucked back into her bun. In fact, a few minutes later when Carol asks for her keys, her hair looks just-brushed shiny and newly coiffed - it's unlikely she would have fussed over her hair at this stressful time.
Continuity mistake: Kerry follows Carol outside and as Carol is holding her gloved hands up in front of her Kerry asks her if she needs anything, and as she replies, "No thanks, I'm fine," her hands are suddenly in her pockets.
Plot hole: Carol and Duncan escape out the back of the shop - but there are no police waiting for them, which makes no sense at all. In reality, with such a situation, the police would have been waiting at the back door. There seems to be no one around at all, as they were able to dash through an apartment. When they finally do run into a cop, he automatically assumes that Carol is an innocent bystander, and not one of the robbers. He couldn't have known this, and in reality, a policeman wouldn't assume it.
Continuity mistake: Carol is putting little bandages on Duncan's face and we see her put 2 on his forehead and 3 on his cheek. The shot changes and now there are 3 on his forehead and those on his cheek have shifted position slightly.
Continuity mistake: When Carol is talking to Angie in the hospital, there is quite a large bloodstain on Carol's sweater, just below her left shoulder. She leaves the room, meets Doug, and without explanation, the bloodstain has disappeared.
Continuity mistake: When Mark is examining Louise, her Barbie changes position several times from shot to shot.
Continuity mistake: Carol sits down at the breakfast table in a normal sitting position, however the next shot shows her with her knee up on the chair, almost up to her chin.
Continuity mistake: Mark is talking to Maggie about getting Louise on the transplant list - he has his arms clasped in front of him with his hands wrapped around his elbows. The shot changes and his right arm is outstretched with his hand on the wall - with no time for the change.

The shot from the hall shows his body from the armpits up is in the hall, but from the shot from inside the bathroom, he is lying in the hall from the hip up. Nobody moved him in the meantime.

Continuity mistake: When Peter goes into Carla's room to talk to her, we see him let go of the door, but in the shot showing him entering the room, he is still holding the door.
One More for the Road - S3-E22
Continuity mistake: In this episode you see Carol and Doug discussing Charlie over phoning and chart-reviewing. Carol is standing beside Doug. Suddenly somebody calls from behind the desk. It's Tommy, a "friend" of Charlie. When he's in the picture, you see Carol sitting in a chair, instead of standing. If she had moved to the chair, she must have done that SO fast, it's impossible.
