They Treat Horses, Don't They? - S5-E3
Continuity mistake: Doug, Mark and Carol are in the lounge having pizza and Doug has just told them he's the new Pedes ER Attending. Carol asks Mark, "Aren't you thrilled?" Mark is holding a paper cup. A Split Second later, his cup's gone and his hands are apart in front of him. Doug says, "He's got his doubts," and now there's a cup in Mark's left hand.
Continuity mistake: When Mark and Elizabeth are coming round the corner to enter the hospital, Mark says, "Remind me again why you invited her to stay with you", and he has one arm around her and in the other hand he's holding a newspaper. As the shot changes when they go round the corner, his arm is no longer around her and both hands are on the newspaper. There was no time for the switch.
Continuity mistake: At the beginning of the show, when Mark and Elizabeth are playing racquetball outside, an extra wearing a top with red sleeves walks behind Mark and disappears to the left. Just then, as Mark says "Romano, he keeps stalling", we see the same extra behind Mark again, heading to the left.
Continuity mistake: Mark comes into the cafeteria with a sytrofoam bowl of something, and when he sits down he starts to take the lid off. The next shot shows him already eating it with a spoon - there was no time to accomplish this so quickly.
Freak Show - S4-E8
Continuity mistake: When Peter comes in to talk to Rodney's father Isaac, the clock on the wall says 11:40 - which it had also read a few scenes earlier when Rodney came out of surgery. After Peter checks the chart and starts to talk to Isaac, the clock has somehow changed to 3:30.
Random Acts - S3-E20
Continuity mistake: Chris Daw confronts Mark in the parking garage with the bill from the hospital for his brother's care. Mark folds up the bill, but as Chris says, "They don't understand", the bill is suddenly open in his hand again.
Continuity mistake: When Peter and Jeanie are outside talking about their test results, there is an extra in a green sweater behind them who covers more ground than humanly possible from one shot to the next.
Continuity mistake: When Vuselich is lecturing Peter about his medical study, and we see Peter's face, his cap is pulled down over the top of his ears. When the shot is on Vuselich's face, we can now see the tops of Peter's ears. It continues like this for the whole scene.
Obstruction of Justice - S4-E9
Continuity mistake: When Doug and Carol are making out in the exam room, their hands are around each other's waists. When Cynthia interrupts, suddenly his hands are around her face.
Ground Zero - S4-E6
Audio problem: When Greene is outside by the river explaining what happened to him, there are a couple of really bad sound edits in the background noise. These are really obvious as the sound suddenly changes, and for some reason the background noise in this scene is very loud, almost as loud as the dialogue; very strange.
How the Finch Stole Christmas - S6-E9
Continuity mistake: When Romano is operating on the heart patient Valerie, Lucy has an instrument inside the patient. The shot then changes so that we can see her face and as she says, "Dr. Romano," there is no longer anything in her hand and she is holding it up waiting for a nurse to pass her an instrument, which she takes, but then the shot goes behind her - she now has a different instrument in her hand, which is in a different position.
Obstruction of Justice - S4-E9
Continuity mistake: Just before Carter is let out of his jail cell, he is examining his cellmate's carbuncle - his patient had a cigarette behind his ear throughout the scene but as the shot goes to show the cop arriving, the cigarette has gone.
Other mistake: When the patient Isadore is brought in, early in the show, he is transferred from the stretcher onto a table, and we can see him deliberately move his right hand out of the way of the other actors moving him. He is supposed to be unconscious and very seriously injured so would not have done this is real life.
Continuity mistake: During the opening credits, when they are working on an unconscious patient, his belt is undone and his fly is down. A couple of shots later, the fly is up and the belt buckled, even no one touched them in between.
Visible crew/equipment: Lucy is shown getting a coat from under the desk. Yosh comes up to her and says something, then opens the door and walks out of the shot. When he opens the door, there is a reflection of a man wearing headphones and holding a boom mic.
An Intern's Guide to the Galaxy - S11-E5
Continuity mistake: When Dr. Dubenko is explaining a chemical reaction to the other doctors he starts to write on the wall. In the first shot he writes the following: 2H+ + H then the camera focuses on his audience and we hear continuing writing, then the camera cuts back to Dubenko when on the wall the following is written: 2H+ -> A. (00:24:00)
Continuity mistake: When Carol and Luka are sitting in Wrigley Field she has a Diet Coke can in her hand. But when she goes to stand up, she leans down and picks up the can of Coke from the floor - even though at no time did she put it down there.
The Peace of Wild Things - S6-E6
Continuity mistake: An elderly patient is grabbing at Dr. Lawrence's hands and crying, "Help me." He pulls his hand out of her grasp, but in the next shot we see him pull it away from her again.
Greene with Envy (a.k.a. Lawrence of Northwestern) - S6-E3
Continuity mistake: When Dr. Lawrence is tending to the patient with the stabbed wrist, the blood splatter on his lapel goes from bright red to brownish and back to red within a few shots.
Continuity mistake: When Doug and Carol are talking in the Pedes ICU, from one angle his head is against the back of the rocking chair, but from the other angle, his head is leaning forward. This continues for the scene, from shot to shot.
Answer: A man named Paul Sobriki comes into the ER in the season 6 episode 13 " All in the Family." He is complaining of headaches, Carter and Lucy decide to rule out meningitis and do a lumber puncture. Paul struggles during the procedure and believes that both doctors are stabbing him. Lucy, who is interested in Psych thinks something is wrong with their patient mentally, but Carter is dismissive of her. She gets cross with his attitude and she calls the psych department anyway to have a look at him. Later on and near the closing 20 minutes or so of the episode, all the staff start playing loud music and get a cake which appears to be blue lol, together to party as it's Valentines Day, Carter returns from somewhere and asks where Lucy is. He goes to check one of the rooms for her and Paul Sobriki comes up behind him in the dark and stabs him twice in the lower back, he collapses and as he falls unconscious he sees Lucy on the ground also. In the next episode, Be Still My Heart (ep 14), Dr Weaver discovers them and all of the staff work to try and save them, whilst Carter survives. Lucy's wounds are fatal and suffers a pulmonary embolism as Corday and Romano try to save her. Carter is grief stricken and feels guilty about the way he treated her, he becomes addicted to painkillers and Benton, Greene all help him and book him into rehab.
