Continuity mistake: When Jack gets an incoming phone call from the kidnappers, he puts the phone to his left ear with his right hand, then in the next shot, the phone is to his right ear. (00:28:05)
Plot hole: Jeremy Garrett is missing and last known whereabouts is a motel in Massachusetts. The Mass. State Police alert the FBI of a "disabled vehicle report" filed the night before. The vehicle's license plate was called in and a tow truck requested. When the truck arrives to remove the vehicle, it is no longer there. Later in the episode it is revealed that the missing person is having an affair with his boss who is also secretly gay. The wife of the boss admits to picking up Jeremy at the motel after intercepting a 1:00 am phone call to her husband hoping to convince Jeremy not to "out" her husband and ruin their "arrangment" style marriage. She admits to stopping along the highway getting out to argue with Jeremy and eventually pushing him down a cliff to his death. The shot of the push has the two standing in the beam of the headlights and only takes about 15 seconds from the two getting out of the car to her driving off after the crime. How could a state trooper investigate a disabled vehicle that is idling with the headlights on first of all and secondly call for a tow truck not only in less time than the vehicle was sitting there but also without seeing them standing five feet in front of the vehicle arguing?
Continuity mistake: In episode "One for All," when they are first at the school looking at the seating chart, Danny states that the teacher said that all the students are in their assigned seats. Malia's name is in the first row second seat, but she is actually sitting in the first row first seat.
Continuity mistake: In episode "One for All," the names of the students on the bottom of the seating chart do not match the names written on the "desks" to show where the girls sit.
Other mistake: Slightly after the opening credits, Jack Malone is investigating the disappearance of Annie Miller and is standing in her mother's bedroom. He turns to Carol Miller's boyfriend and asks to "speak to Mrs. Malone alone." The subtitles correctly have 'Miller' written, but when Anthony LaPaglia says his line, the two words rhyme. (00:06:45)
Plot hole: At The Beginning Sam stated that the missing teacher comes to work by subway every day. But in the flashback of his conversation with the drug dealer, he's about to get into his car. (00:13:00)
Continuity mistake: When Martin recognizes the bully in the arcade, this kid just passes two pinball machines. When Martin goes after the fleeing kid, he passes just one pinball machine. The left machine has disappeared. (00:17:45)
Revealing mistake: The left pinball in the arcade (which later disappears) has the body of a "Star Trek TNG" machine - but the backglass is purely fictional. (00:17:45)
The Season - S2-E22
Continuity mistake: At the end of the show when Jack tells his wife that he wants to go with her and he hugs her; at The Beginning of the hug the wife has a champagne bottle in her right hand, but when they release each other from the embrace, Jack is holding the champagne bottle in his hand.
Factual error: After the two women found out the guy was dead, there is a truck with the writing "K-9 Unit" on the front, but it is not backwards as it should be.
Continuity mistake: When the professor stands up in the holding cell after Jack tells him that someone saw Skye at 10:30, he slams his chair back causing it to tip over. In the next shot the chair is upright, then when he goes to sit back down, it is again on its side.
All for One - S5-E4
Visible crew/equipment: When Malia points the gun to the dealer, for a second you can see two finger tips at the right edge of the screen. There is no other in the alley and it can't be the dealer because the angle would be wrong. (00:29:20)
