Vessel - S5-E22

Visible crew/equipment: Episode 22, Season 5 - "Vessel": Lex returns to the mansion, after being abducted, with his new powers. When he shows Lana by shooting his hand, they start talking by the stained glass window. The reflections in their eyes are supposed to be the light coming through the window, but are actually from a white screen used for lighting. The reflections in their eyes are rectangular to confirm this.

Visible crew/equipment: As Jonathan is being attacked by Hercules you can see that Jonathan, or his stunt double, has on a arm cuff that is used for protection of attack dogs.

Zod - S6-E1

Visible crew/equipment: Martha and Lionel are in the barn when everything starts to fall apart. The camera shot changes from them to a boat falling from the rafters. As it falls there is a visible production crewmember hiding next to the window, holding a hay bale. (00:28:15)

Asylum - S3-E9

Visible crew/equipment: As Clark is ambushed inside Belle Reve, in shots focusing on Lex as he is being beaten by the twin and again a few seconds later, there is a dark object in the upper right corner of the shot. There is nothing in this scene to explain it and it moves with the camera so it is probably some piece of filming equipment. Regardless of what it is, it should not be in the shot.

Legacy - S3-E17

Visible crew/equipment: As the FBI search the Kent's house, you can see the shadow of the camera as it moves from the kitchen to the hallway next to the stairs. The shadow appears on the right side of the arch between the kitchen and hallway. (00:17:50)

Skinwalker - S2-E10

Visible crew/equipment: After Joseph gets locked out of the construction site, the worker, who locked him out, walks away. All the other workers at the site are gone, but when the shot returns to Joseph you can see the shoulder of someone standing between Joseph and the gate even though there should be nobody there.

Memoria - S3-E19

As they walk across, and eventually walk under another building, as they cross the courtyard, you can see the shadow of a large reflector board moving along with them as they walk.

Nemesis - S6-E19

Visible crew/equipment: After Clark and Lex get out of the tunnels, and the explosions follow them out, we see Lana and Chloe arrive in a dark car. When they get out of the car, the cameraman can be seen reflected in the door. (00:28:00)

Skinwalker - S2-E10

Visible crew/equipment: After Kyla dies the scene goes to the construction site as Martha and Lionel get out of a SUV. As Martha gets out you can see the camera and film crew reflected in the window of the SUV.

Lucy - S4-E16

Visible crew/equipment: Just after Becker starts to chase Lucy there is a shot looking up the hill towards the lodge where she started skiing from. To Lucy's left you can see a fresh pair of ski tracks being made by whatever the camera is on. You can tell these are fresh tracks because there is fresh snow being kicked in the air because of them.

Red - S2-E4

Visible crew/equipment: After the scene transition from when Clark throws Jonathan into the truck after being exposed to Red Kryptonite, the camera pans around Jonathan with his shirt off; on the left-side of the screen we see a man with sunglasses on who has no earthly right to be there just randomly on the Kent Farm. (00:16:23)

Skinwalker - S2-E10

Visible crew/equipment: While Clark and Pete are riding dirt bikes, you can see a crew-member in a green jacket in the bottom-right-hand corner of the screen in the shot where they go off the jump. In the next shot he has disappeared.

X-Ray - S1-E4

Visible crew/equipment: After Martha finds the money in the antique shop she tells the other woman she needs to be more careful, as they are walking in the room there is a boom mike shadow moving.

Upvote valid corrections to help move entries into the corrections section.

Suggested correction: You see a person walk behind her in a corridor. Since it is very obvious and occurs during a short cut where nothing else is happening, it is deliberate. A fundamental rule on film sets is that once "action" is called, no-one moves if they don't have to. If a crew member had walked through the shot like that, they would have done a new take or just use a different segment of the shot.

Whisper - S3-E10

Question: In the episode, 'Whisper', there is a character that makes a very high pitched feedback like noise. Is this a character that was in the old comics and can anyone tell me how this guy does that noise?

Answer: No, there is no such character in the comics. He's a "meteor-freak" like all the other baddies on the show. The sound has something to do with him vibrating his vocal cords very fast to create a high-pitched squeal.
