The Daleks - S1-E2
Revealing mistake: There are numerous scenes where the background Daleks are blown-up photographs. The most obvious is in episode five when The Daleks are discussing the radiation drugs.
Visible crew/equipment: In episode one, when the Doctor is near the pyramid, Ian's leg can be seen just before the shot ends, as the consecutive shots used the same section of wall.
The Daleks - S1-E2
Continuity mistake: The Dalek the crew ambush in episode three has a magnetic block in its sucker and, after the attack, a mud-covered eyestalk. At the beginning of episode four, the eye-stalk is visibly clear of mud, yet is later seen plastered with mud once more.
Continuity mistake: In episode one, Barbara activates her travel dial only seconds before the others. But in episode two, she's been at Morphoton for quite some time - long enough to change clothes, pick fabrics and get food.
The Daleks - S1-E2
Revealing mistake: When the Thals are using sunlight to blind the Dalek cameras in Episode Six, wide shots of the Dalek city show it moving back and forth from bad matte combination.
The Daleks - S1-E2
Visible crew/equipment: Toward the end of episode one, when Ian is wondering where Barbara is, the shadow of a boom mic moves over the door behind him.
Visible crew/equipment: In episode one, when Ian is first seen inside the pyramid, someone stumbles through the background. Some have claimed this is Barbara, but it happens too fast to tell.
Visible crew/equipment: Part 4, A Race Against Death: The Doctor chides the First Elder about Ian's illness and says, "I must have chemicals and equipment; his death will be your fault and yours alone." Then, just as the Second Elder appears and says, "Sir," the boom mic (with white end) swings up between the two Sensorites, and we also see the diagonal boom pole moving at the left side of the screen.
The Daleks - S1-E2
Revealing mistake: The Dalek that Ian hides from in Episode Seven has a bad castor. The flopping of one of its wheels is audible. (00:08:20)
Visible crew/equipment: In episode one, when a Voord falls through a pyramid door, a stage hand can be briefly seen on the other side.
Other mistake: In episode one, when the crew wonders what The Sensorites will do to them, the camera zooms in on the Doctor as he says, "or kill us?" It then jerks with an audible click as though hitting something.
Continuity mistake: Susan's socks disappear during the opening credits.
The Aztecs - S1-E6
Revealing mistake: In the final fight, Ixta sends Ian staggering into the altar, which slides out of position across the floor. Also, at one point the edge of the raised set they're fighting on is visible briefly.
Audio problem: During Hartnell's big speech about the creation of a solar system, a cough can be heard from off-stage.
Continuity mistake: Part 6, A Desperate Venture: The new Second Elder threatens Carol and has her write a note to John. When Carol hands the finished note to the new Second Elder, this close-up dissolves into another close-up of the note being held by Barbara. Carol's note is quite different in both of these close-ups.
Visible crew/equipment: When Susan runs out into the console room in a panic, the shadows of two crew can be seen on the wall.
The Daleks - S1-E2
Other mistake: Why does The Daleks' prison have beds? The Daleks don't need them and clearly state that they have no idea what the Thals look like, speculating that they are "horribly mutated".
The Aztecs - S1-E6
Other mistake: During the sacrifice scene, the camera bumps into some scenery and suddenly lurches.
Visible crew/equipment: Part 5, Kidnap: In the Sensorite lab, the Scientist shows the Doctor and Ian a collection of things left behind by the humans, who were killed in the spaceship explosion. When Ian flips through the family snapshots, the shadow of the boom mic passes over Ian's forehead. It happens again just as Susan blurts, "The City Administrator, of course!"
Visible crew/equipment: Part 4, A Race Against Death: While Susan is with Ian and the First Elder, Ian tells her, "The Doctor must have run into a snag..." When Susan replies that she's going to find out why, just as she stands up a boom mic becomes visible at the top right side of the screen, and it's pulled up out of view. Then, as the First Elder says, "I will have you conducted to the laboratory," the shadow of the boom mic is on his head.

Answer: TARDISes are generally available for properly authorised use on Gallifrey; they're not usually assigned to a particular Timelord on a long-term basis. The Doctor stole his when he left his homeworld.
