When Luke is being pulled through the broken window he's holding his lightsaber in his right hand, then it disappears from his outstretched hand in the wide shot, but in the next closeup his lightsaber is suddenly clipped to his belt at his left side. (00:35:15)

Partial Terms of Endearment - S8-E21
Continuity mistake: In this episode, Lois is shown to have dated Naomi during college. But in the episode "Meet The Quagmires", it was established that Lois and Peter were already dating when Lois was eighteen. She accepted Peter's marriage proposal at that age. She would have been engaged and possibly married during college, not dating other people.
And Then There Were Fewer - S9-E1
Character mistake: When driving to the manor, Lois wakes up and says "Peter, look" as if she knows they are there. She could have not been able to notice this since the trees are blocking all view of the manor. (00:03:00)
And Then There Were Fewer - S9-E1
Other mistake: As Joe is driving up to the house, just before coming to a complete stop, you can see that his window is completely closed. After stopping, Peter says "Joe?" Joe says "Peter?" Joe's window is now fully open. Trouble is, Joe seems to be completely surprised by Peter standing there which implies he never saw Peter even after stopping. His window has mysteriously opened immediately. (00:04:05)
And Then There Were Fewer - S9-E1
Continuity mistake: When Carl mentions Labyrinth to Tom Tucker and Diane Simmons, Meg can be seen at the other side of the table sitting in front of Joe. But when we next see Meg a few moments later, Doctor Hartman is now sitting where Joe was. (00:07:25)
And Then There Were Fewer - S9-E1
Continuity mistake: Just after Stephanie is killed, there is an outside shot of the manor, where the weather is normal. Just 20 seconds later when they run outside to try and escape, it is suddenly pouring down. (00:10:40)
And Then There Were Fewer - S9-E1
Continuity mistake: The interior layout of the dining room is visible in many shots, specifically the wall and windows behind the arm chair at the head of the table. After Stephanie is murdered, when Mort shouts to run for their lives, the crowd exits the dining room and they turn right, then everyone runs for the double front doors, opposite the staircase. This means the windows behind the arm chair at the head of the table are at the back of the house, but later when they capture Tom behind that chair, the shot moves backwards through the window and it's now at the front of the house, beside the double front doors. (00:10:50 - 00:37:30)
And Then There Were Fewer - S9-E1
Continuity mistake: When James returns to the lounge to find everyone there, they all accuse him of killing Stephanie, so they walk over to the dining room with James at the front followed by Tom and Priscilla behind him and Joe and Doctor Hartman behind them. But when they enter the dining room, Tom and Priscilla are no where to be seen and it is now Joe who is behind him. Also in that same scene, several other characters change position. The people who are in the room are Peter, Chris, Brian, Quagmire, Joe, Bonnie, Diane, Doctor Hartman and Seamus and the rest of the guests aren't seen. So when they enter, Diane and Chris are at the back followed by Seamus, Quagmire and Bonnie in front of them, followed by Doctor Hartman, Peter and Joe at the front. There is then a shot of the chair completely empty. Yet in the next shot, Brian has suddenly appeared next to Joe out of nowhere. Then in the following shot, Diane is still at the back but now more closer to Bonnie and both Bonnie and Quagmire have changed positions and Seamus is now at the back and Chris is suddenly next to Quagmire at the edge of the right hand side of the screen and Peter is now behind Joe. Then the shot cuts to James and the shot cuts back to guests. But now Peter has appeared next to Doctor Hartman again and now in front of Joe and not behind him. Also Diane is now no longer at the back but in the middle towards the left of screen and no where Bonnie and Seamus is now partially in front Bonnie. Then when the shot cuts to James saying he is a member of MENSA and a huge hit with the ladies, there is no sign of Brian standing next to Joe. Then in the shot before the lights go out, Diane is now at least ten inches behind Bonnie and Lois has appeared behind Seamus out of nowhere. Then by the time the lights go back on again, Lois is now in front of Joe and all the other guests have appeared in the dining room. Then when Seamus says "Look", there's no one in front of him even though loads of other people were there in front of him before the lights went out. Then the shot zooms out and we can see in the shot are Doctor Hartman, Priscilla, Joe, Brian, Carl, Mort and Tom. Then when Joe says "Nobody touch the knife", it's now Seamus, Bonnie, Quagmire, Lois and Derek in the shot. Then when we see Peter and Chris in the shot, the shot zooms were Joe appears we can see Quagmire, Lois and Bonnie behind Peter even though they were way in front of them a couple of seconds ago. No way could they have all moved that fast, not with all the other people in the way. (00:13:40)
And Then There Were Fewer - S9-E1
Continuity mistake: When Mayor West panics, Peter shakes him and he falls against a shelf. The plate on the end of the shelf falls off and smashes on the floor. But when Brian points out the gun is there, it is now the middle plate that fell off and the plate on the end of the shelf is intact. (00:15:25)
And Then There Were Fewer - S9-E1
Continuity mistake: When Joe concludes that Stephanie was in the wrong place at the wrong time, the dining room table is still set with plates, wine glasses, etc, but then Joe tells everyone to look around for more clues, and in the next overhead shot as they all exit the dining room the table is bare. (00:15:55)
And Then There Were Fewer - S9-E1
Continuity mistake: Peter knocks out Joe during the search in the hallway. Yet when everyone is fighting about the motives, there is a wide shot of the fight but Joe can be seen fighting behind a couch with Doctor Hartman and Derek. He can't have regained consciousness, because he doesn't regain consciousness until later in the episode. (00:19:20)
And Then There Were Fewer - S9-E1
Continuity mistake: As soon as the secret passage opens, the book that caused it to open disappears off the ground almost instantly. (00:19:25)
And Then There Were Fewer - S9-E1
Continuity mistake: When the guests walk through the secret passage, the order of the guests change between shots. First, it's Peter, then Chris, then Carl, then Doctor Hartman, then Seamus, then Quagmire, then Lois. But in the shot of the passage, the order is Peter, then Chris, then Carl, then Lois, then Consuela, then Doctor Hartman. Then when they reach the secret room, the order is Peter, then Chris, then Carl, then Lois, then Bonnie, then Quagmire, then Derek, then Jillian. (00:20:00)
And Then There Were Fewer - S9-E1
Continuity mistake: In the first shot of the secret room, there is a desk with a lamp, a cup with pencils in and two sheets of paper on. In the next shot, a blue book suddenly appears on the desk. (00:20:15)
And Then There Were Fewer - S9-E1
Continuity mistake: When everyone enters the secret room there's a tall lamp (which is turned on) on the desk, but after Peter finds the book, just as he asks Bonnie about being punched in the throat, the lamp has vanished in this shot. (00:20:15)
And Then There Were Fewer - S9-E1
Continuity mistake: While everyone is in the secret room Peter finds a blue book on the desk and picks it up to read. Peter's holding this book in his hands when he asks Consuela what happened, and just as Consuela tells him about her nephew, we see another blue book on the desk beside Peter. There should be only one blue book - the one in Peter's hands. (00:20:40)
And Then There Were Fewer - S9-E1
Continuity mistake: When they notice Muriel has disappeared, everyone runs back up the secret passage. As they run out the room, the order of the guests are: Peter, Diane, Chris, Lois, Carl, Doctor Hartman, Meg, Jillian, Brian, Quagmire, Mort, Derek, Seamus and Mayor West is way at the back. Yet as they run through the passage the order is: Peter, Lois, Tom, Quagmire, Diane, Chris, etc. But as they come out of the passage, the order is: Peter, Lois, Tom, Diane, Chris, Bonnie, Meg, Quagmire, Carl, Mayor West, Doctor Hartman, Mort, Seamus and Jillian is way at the back and not Mayor West since he was last seen exiting the secret room. Also Derek nor Consuela were seen in the room just as they ran out, yet when they magically appear when they are doing the groups. (00:21:50)
Answer: Lacey Chabert was the actress who originally voiced Meg in early episodes. She left of her own accord due to being in school and other acting work; Seth MacFarlane has stated there was no tension with her leaving, but it makes for a handy joke for the show.
