Continuity mistake: After Iron Guts Kelly dies in Margaret's tent, Frank Burns visits Margaret, suspecting something. Frank asks why there is a general's star on her cot. In the ensuing 'back and forth' dialogue, Margaret's hair goes (in close ups) from nicely combed, to frazzled, back to nicely combed again (and in a different light).
Other mistake: When Alan Alda and Mike Farrell are walking into frame in the scene where Hawkeye and BJ discover the huge shipment of tongue depressors, Alda's feet are visible for about two seconds and he has bright blue early '80s shoes on.
Continuity mistake: When Frank is trying to examine the Colonel, in some shots Buzz's robe is open and in others it is tied at the waist. (00:10:50)
Factual error: During this episode, Sergent Klinger gives a Hershey Chocolate Bar with Almonds (although the 'Hershey' name is obliterated on the bulk box as well as the bar itself, there is no doubt as to what it is) to a wounded North Korean soldier, who accepts it and turns it upside down, revealing a Universal Product Code bar code. Bar codes were not used on products until some 20 years later. (00:12:20)
Visible crew/equipment: At the end, after Father Mulcahy says goodbye to Sarah, when it cuts to a wideshot we can see two actors' rubber T-marks on the ground. The first is behind Father Mulcahy's feet, a white T-mark (albeit dirty) where Sarah had been standing, and the second becomes visible when Klinger runs over and bumps into Kellye, he kicks up some dirt which causes the black rubber T-mark to flip up off the ground.
Visible crew/equipment: After Harper, the JAG, has Trapper and Hawkeye sign and attest to the blood report, when Radar enters Henry's office we can see a crewmember's hand near the floor between the two doors, preventing the closing door from swinging back and forth. (00:22:20)
Good-Bye Radar: Part 2 - S8-E5
Visible crew/equipment: After Radar tells Colonel Potter that he decided to go home after all it cuts to Hawkeye walking into the office, and just as Hawkeye opens the door we can see the top of the set.
Are You Now, Margaret? - S8-E2
Visible crew/equipment: When Williamson and Hawkeye walk into the Swamp and stand near BJ's cot, the edge of the tent set can be seen at the top, left side of the screen.
Continuity mistake: When Henry, Trapper, and Hawkeye get into the jeep with Radar notice the style of jeep and its number is 16930382, but when they're driving to the chopper pad the style differs - things such as hood, antenna, etc, and its number is 11172419. Then when the chopper takes off the jeep's style is back to the original jeep they were in.
Visible crew/equipment: When Margaret's in her tent reading just as Hawkeye walks in to give her the physical, the chalk T-mark can be seen on the floor.
Continuity mistake: When Hawkeye and Lt. Johnson are sharing Hawkeye's bottle of "wine" there's a closeup of Johnson's hand holding the glass, and in the closeup that hand has short nails, whereas in all other shots Johnson has long nails.
Visible crew/equipment: When the nurse tells BJ that they're having a problem with Rice using his sales pitch in post-op, just as the nurse and BJ start walking over to Rice we can see the curved chalk foot mark on the floor.
Visible crew/equipment: After Klinger gives the envelope with the forged discharge papers to the driver going to I Corps, just as he walks through the doorway we can see the two actors' tape marks on the ground, where the driver and Sidney were standing when they arrived.
Communication Breakdown - S10-E6
Visible crew/equipment: While Colonel Potter and Margaret are with Charles, a nurse sees them and announces the presence of a newspaper, then when everyone gathers around Charles while he sets the reading rules, there's a man with blondish shoulder length hair and a beard, wearing a blue button down shirt and white pants who's standing in the background, partially hidden by the signpost.
Factual error: After the opening credits it cuts to Klinger at Mail Call, and Father Mulcahy's reading Ring Magazine which is dated December 1969, 16 years after the Korean War ended.
Visible crew/equipment: While in the mess tent, after Margaret's grossed out by the smell and walks away we hear Charles start playing again, and when Colonel Potter says, "Just make sure the three of you work this out fast," in this shot we can see the actor's T-mark on the floor between Potter and Hawkeye, where Margaret stood a moment ago.
Dear Mildred - S4-E7
Visible crew/equipment: In the scene where BJ, Hawkeye, and Radar are in the motorcade talking about the horse, Radar is talking about how he doesn't want to give away the horse. He is standing in front of some barrels and when he moves, you can see some of the crew, the horse owner, and what looks like another horse in the background for 1 second.
The Trial of Henry Blake - S2-E8
Visible crew/equipment: After Henry's cleared of all charges, when Margaret drops the crumpled paper and storms out of the room with Frank, we can see chalk marks and a T-mark on the floor where Frank was standing.

Continuity mistake: When Lieutenant Willis wakes up in the Swamp and puts his shirt on he's wearing his dog tags, but when he tosses the papers and puts on the rest of his clothes the dog tags have suddenly vanished, then they reappear, vanish, and reappear again.
Answer: It is actually Cencompac for Central Command of the Pacific. It is like the headquarters for all military activity in the Pacific Region (Japan, Korea, Okinawa, etc. The newer version of that is United States Pacific Command (USPACOM).
