Continuity mistake: Inside the laundry cart, Kindaichi asks Longtao where does he bring the linens. The cloth on top of his head slips behind him, but this happens again in the next shot; it's back on his head and drops all the way down again. (00:09:35)

Continuity mistake: When Kindaichi falls from the cart, the boxes around him are placed in a way that it's not just incongruous continuity-wise from shot to shot, but is also nonsensical; in one shot for instance he has a box through each leg, but since Longtao smuggled him out inside them, both legs had to be inside the same box; the cardboard was surrounding him. Other mistake: The two survivors from 18 years prior approach the building with nothing on their person, then suddenly Matsuoka has a flashlight, which he gives to Shinomiya as he goes inside alone. Kindaichi and Longtao arrive, completely unprepared to any dungeon crawling, and the three of them go in with yet another flashlight. (00:13:50)

Plot hole: It's unreasonable, nor is explained in the denouement at the end of this arc, how did the murderer, who stayed behind everyone else on purpose, get to Shinomiya who was ahead of everyone, went inside the cave before everyone else and didn't have a reason to hide, cooperate with him or anything of the sort.

Other mistake: When Kindaichi rushes to Longtao's rescue, he is not carrying the flashlight with him. He's in a deep cave bunker with no light at all (plot point mere moments earlier), he should be blind as a bat. In the following episode he even searches for clues in the cave, again without the light.

Continuity mistake: When Kindaichi holds the army knife picking it up from the ground, it has two stripes of blood across the blade, just one stripe when Detective Li picks it up.

Continuity mistake: After the flashback to the event of 18 years earlier, there's a side view of Matsuoka's face who says that they feared Jun would be still alive. The stack of papers on the desk is more towards the center of the desk than it is in the rest of the scene. (00:08:05)

Plot hole: The crowd freaks out when Matsuoka's body is found and Kindaichi happens to be standing in front of it, gun in hand. That's all sorts of silly though, not only because the killer could have never predicted that turn of events, but also because Kindaichi roamed the streets looking for Matsuoka, gun in hand the whole time (it's a different gun from the one Matsuoka is killed with), and it's a girl screaming that draws everyone's attention to the murder. What about the two gunshots? Everyone in the background was entirely unfazed. It also creates a separate issue in the ending, since the plan to frame Kindaichi was supposed to be 'perfect' but the different gun would have spoiled it. (00:08:35)

Other mistake: Kindaichi in this episode watches the footage of the events in the previous episode from Saki's handicam. Saki was running with the camera and the camera was jerking up and down a whole lot in those animations, but here the motion is very subtle. Li also was standing facing straight away from the door, parallel to it, but here he's more turned to the left.

Plot hole: Kindaichi bases his solution on a fact (there was a certain object on the elevator together with the victim) he couldn't know about, since Li did not mention it (and if he did, he would have been on the right track to solve the mystery himself).

Continuity mistake: At the beginning of the episode, the distance between the suspects and the proportions are noticeably different from the end of the previous episode, and between shots within this very episode too.
