Just Let Go - S1-E4

Audio problem: When Devon's mom is interrogating Jake, when Jake says "And then we'll be done?", his mouth movements don't match the words in the slightest. (It looks more like he's saying "I don't know.").

Revealing mistake: When Lexy falls through the railing, there's a few shots where you can tell she's been replaced by a (probably adult) stunt double. She suddenly seems quite a bit taller, and noticeably has more muscle tone in her legs.

Continuity mistake: When Lexy asks if Jake really wanted her to die, her flashlight goes from pointed down to pointed up towards Jake, then back down again between cuts. This mistake repeats several times throughout this sequence.

Continuity mistake: When Jake and Lexy are exploring the burned-down house, at one point, Jake turns about 180 degrees instantly between shots. (It's shortly after they go into the room that's incredibly damaged and Lexy says "Oh my god.").

Revealing mistake: During the flashback, when Charles shows the kids the body of the janitor, in the first close-up of the Janitor's face, you can see a pulse in his neck. (The next time you see his neck, you can also see a slight pulse, albeit, it's much, MUCH subtler in this shot.)

Other mistake: When Jake and Lexy depart the hospital, she's wearing a blue shirt and pants. When they get to her house, she's suddenly wearing a white shirt, denim jacket, and a skirt. But the indication is that they went directly to her house from the hospital, so where'd she get the change of clothes from?

Continuity mistake: When the woman grabs the ball from near Jake's foot at the beginning of the episode, in the first shot, you can see she starts to transfer the ball from her left hand to her right, but in the next shot her hands are suddenly far apart.

Continuity mistake: When Lexy talks to her parents near the beginning and her mom says, "Suddenly you care about your little sister? That's funny," her mom goes from facing her father to facing her between cuts.

Continuity mistake: When Devon is talking to his mum at the beginning of the episode, the way his headphones are sitting in front of him slightly changes. Sometimes the cord is sort-of sticking up, sometimes not.

Audio problem: When Jake is talking to Lexy at the beginning of the episode, when he says "But, you didn't see him die?" his lips don't quite match what he's saying, revealing the line was dubbed in post.

Goin' to the Chapel - S2-E7

Trivia: SPOILERS: In season two, episode seven, when Father Bryce explodes and it is shown over and over from a multitude of angles, it is a direct homage to the 1978 film "The Fury." At the end of that film, a character explodes in a similar fashion, being shown over and over again from different angles. Series creator Don Mancini has discussed how much he enjoys "The Fury" in the past, and has thrown in some subtle references to it in the "Chucky" franchise.

Answer: They were blamed for the death of Jake's foster brother, which was deemed a terrible accident, and they were sent to the reform school as a punishment. Presumably, when they had to leave the reform school given what happened, they were instead given community service as punishment.
