Continuity mistake: When Ghost Rider crashes his motorcycle out of the building with Talbot in tow, he is holding Talbot around his midsection. When the shot changes to them in free fall, he is gripping Talbot by his collar. When they land on the street, he is back to holding him around his midsection.
Other mistake: When Bruce Banner is attempting to rescue the nun from the fire, a beam falls from the ceiling and hits him in the head, triggering his transformation into the Hulk. Just before he starts his transformation, a set of green glowing eyes appear above his head. It appears the animators didn't properly sync the glowing eyes onto Banner's face.
Continuity mistake: When Talbot arrives to attack Hulk, he is wearing a hat. When he falls out of the helicopter, the hat is missing. When he lands on the roof, the hat is back on his head.
Revealing mistake: When the Hulk is sitting on the edge of a building, Talbot suddenly appears in a helicopter to ambush him. If you watch closely, you can see the helicopter originates from behind Hulk instead of rising up from behind the building.
Continuity mistake: When Hulk jumps up onto Talbot's helicopter, it changes color from an army green to blue.
Continuity mistake: When the Hulk lands in the streets of Chicago after his battle with Ghost Rider, he is in the middle of the street. When the shot changes, he is suddenly next to the curb and a mailbox.
Helping Hands, Iron Fist - S1-E4
Continuity mistake: Early in the Hulk's fight with Iron Man, he punches Iron Man onto the hood of a red car and then throws a blue car at him. Iron Man disappears from the screen as soon as the blue car hits the red car, and then can be seen either back on the hood, or instantly inside the car. If he is still on the hood, then he disappears from the screen once again when Hulk throws the car.
Factual error: The moon in the night sky in this episode is far too large.

Continuity mistake: When Dr. Langkowski is talking about solving the mystery of gamma mutation in humans, a couple pens appear in his shirt pocket.
Factual error: The opening shot of this episode incorrectly depicts the Lincoln Memorial Reflecting Pool to be located between the Washington Monument and the U.S. Capitol building. In reality, the reflecting pool is located between the Lincoln Memorial and the World War II Memorial, with the Washington Monument roughly 1/5 of a mile east of the World War II Memorial, and the U.S. Capitol nearly a mile and a half east of the Washington Monument.
Helping Hands, Iron Fist - S1-E4
Continuity mistake: After Rick Jones finds Bruce Banner among the overpass wreckage, Bruce starts transforming into the Hulk and grabs some gray rubble, which immediately turns the same color as his flesh.
Continuity mistake: When Bruce and She-Hulk are hitchhiking, the sign for New York behind them changes size and lettering. When it is first shown, it is significantly taller than both of them with the lettering reading "NeW York" (sic) in a thin and long font type. When the shot changes, the sign is much shorter and the lettering now reads as "NEW York" in a more block-lettering style.

Continuity mistake: When Doc Samson is carrying the cylindrical tank for the nutrient bath, he is holding it from the bottom rim. When the shot changes, he is holding it from the outside.
Factual error: When Hulk lands on top of Mount Rushmore, Teddy Roosevelt's glasses are missing.

Continuity mistake: When Dr. Langkowski climbs into the large test tube to run the experiment on himself and Banner, he uses a hatch wheel to seal his tube closed. When the shot changes, the wheel has disappeared.
Answer: The Hulk is a giant brute who smashes everything in his path when he is angry. In Ang Lee's "Hulk," General Ross, played by Sam Elliot, explains it to his daughter.
