Revealing mistake: When the Gamma Warriors are running towards Hulk on the rooftop, you can see two creases going vertically across the screen, likely from the edges of the animation cells. (00:20:18)
Continuity mistake: When She-Hulk and Betty begin climbing up the elevator shaft, at first they are climbing up rope. When the shot changes, the rope now has the appearance of elevator cables. (00:15:14)
Continuity mistake: When the model removes the building schematic from the circuit breaker door, the schematic is suddenly drastically smaller when the shot changes. (00:14:56)
Continuity mistake: A handle appears on the door of the laundry chute that the models use to escape the dressing room. (00:11:42 - 00:12:23)
Continuity mistake: When Gargoyle is saying "Seal the doors," his left eye is bulging while his right eye is normal size. It's supposed to be the other way around. (00:09:30)
Other mistake: When Bruce Banner is loosening his bow tie, he says it's cutting off the circulation to his head. If you look closely, you can see that it was loosely wrapped around his neck to begin with.
Continuity mistake: When She-Hulk and Bruce's limo is approaching the building, you can see the car slowing to a stop. When the shot changes to inside the limo, you can tell by the light glares passing by the window behind Bruce that the limo is now moving faster.
Other mistake: The reporter in the opening scene mentions that the Chateau de Shae is 80 floors. The camera then pans up the entire length of the building, and you can see it is nowhere near 80 floors high. It's only roughly half that size.
Other mistake: When Lou Ferrigno is walking away from the reporter, his movement suggests that he is walking on a downward slope, even though he is on level ground. He also disappears into thin air just before he would have stepped out of view.
Other mistake: The reflection of the spotlights in the water in the opening shot does not match the movement of the spotlights themselves. (00:00:01)
Other mistake: When the Leader and his minions are escaping to the rooftop, the bomb detonator says there are only 30 seconds left. After Leader escapes and Hulk goes back into the building to get the bomb, the detonator says 10 seconds remain. While there is an argument to be made in movies and television not being strict with real time, in this instance, far too much happened onscreen for only 20 seconds to have passed. The heroes confronted the villains, one villain tried to talk a hero into coming with them, the police arrive on the scene, a battle happens on the rooftop, the villains escape with the police in pursuit, etc.
Continuity mistake: When Gargoyle is tempting She-Hulk with the ransom money, his right thumbnail turns from dark gray to the same blue as his outfit. (00:20:09)
Factual error: When She-Hulk loses her grip on the elevator cable, she catches onto the structure on the walls, but Betty loses her grip and falls several floors before She-Hulk can fall after her. A couple of shots later, She-Hulk has already caught up with Betty, even though there wouldn't be enough distance for She-Hulk to have reached terminal velocity to catch up with Betty.
Answer: I believe it was mentioned in an issue of Wizard Magazine that the show was cancelled because of low ratings, and reportedly the show's creative shift in the second season had a part to play in the low ratings, as the changes alienated fans.
