Character mistake: The Eagle Koenig flies is called Eagle One. However, the number on the door behind him is plainly a four, meaning Eagle Four.
Suggested correction: This happens a lot. The number on the door is not the number of the Eagle, it's the number of the command module. The super structure and drive systems are Eagle One. Even then, there has been several Eagle Ones destroyed or lost. Eagle One is assigned its designation by the head of Recon, Alan Carter.
Other mistake: At Waste Area 2, as Nordstrum throws Steiner, his visor flips open.
Visible crew/equipment: In the beginning when the camera enters room 3C with the plants, a shadow of the cameraman can be seen on the door frame. (00:02:05)
Revealing mistake: As the Eagle lands on the new planet, the wires for flying the model can be seen.

Continuity mistake: As Paul calls to all sections, he's standing over a console, but on the screens, there's a wall directly behind him, and the shadow of his head can be seen over his left shoulder.
Continuity mistake: As the experiment recreation progresses, and Helena identifies the spirit, the spirit appears with damage on the left side of its face, and a yellow sleeve on its right arm. When the camera is close up to the spirit, the damage and yellow sleeve change sides.

Continuity mistake: The number on Kelly's suit was 4. As Paul went to go out after Kelly, you see the number 4 on the control panel, then when he nears Kelly, it's changed to 3, suggesting the shot of the exiting person was re-used.
Continuity mistake: As the scene in Main Control gets blurrier as Koenig spins around, the part in his hair is first on the left, then on the right, then back on the left.
Other mistake: As the camera pans around to look in the cockpit, the wound on Koenig's head is easily seen as a bad blending job. The edges of the prosthetic are visible.
Other mistake: In the beginning, the Eagle was moving away from the alien planet, and as such, should have crash landed facing away from the planet. However, it was facing the direction it came from. If it traveled a fairly straight line, (and the trajectory suggests it did) then it should have crashed with the rear end facing the planet.
Continuity mistake: As Helena is tapping on the key panel, the row of black keys goes from second on the right to second on the left and back, showing the shot was reversed.
Revealing mistake: As the "dead" companion lies in the coffin, you can see him breathing.
Revealing mistake: As the camera looks at the exhaust port of the probe's engine, there's water dripping off the bottom, showing the stream is water vapor.
Other mistake: Every time someone calls, either on the comlinks, or the wall panels, there always appears to be a blank wall immediately behind the person on screen, even when the camera cuts to the person calling.
Continuity mistake: As the alien struggles with the security personnel, it's easy to tell the alien actor and the stunt double. The hair color and style change between distance and close up shots.
Continuity mistake: The panel Tony hits with a tomahawk in his quarters first appears to be molded plastic, then in a close up, appears to be painted wood, as the wood grain where the panel above is smooth.
Revealing mistake: As Koenig tells of the attack, his feet are passing by the "dead" Mike Baxter. As they pass, Baxter's eyes flicker for a second.
Revealing mistake: Every time "red alert" is on the screen, it's easy to see the screen is an overlay with "red alert" cut into it.
Revealing mistake: As the Voyager probe lands, the control wire lowering it is visible.
Revealing mistake: As the Eagles take off from Piri, the wires used on the models to "fly" are easily seen.
