Trivia: In the beginning, when Kimberley wakes up suddenly, listen carefully. You can hear Tony Todd (the guy at the morgue) saying "Kimberly".

Final Destination 2 (2003)
Continuity mistake: Look at the newspaper photo of Clear before she committed herself - she has relatively dark hair. Definately not a natural blonde. Yet current Clear has much, much lighter blonde hair. We saw that she was blonde at the end of Final Destination - yet, her darker roots aren't particularly prominent, showing that during her time in the institution she must have bleached her hair at least once. For someone who's paranoid about death, she sure doesn't seem to care about the potentially acidic effect of having peroxide on her head. [Someone asks this in the commentary on the DVD, and the response is "she was a natural blonde, she dyed her hair brown in the first movie." This stands up, as it explains why there were no brown roots at the end of the first movie.]

Shanghai Knights (2003)
Continuity mistake: During the library and museum scenes particularly, the heights of candles and the amount of light given off by them varies significantly from shot to shot. This is because Jackie Chan fight scenes tend to be filmed over a matter of weeks.

X-Men: Evolution (2000)
Mainstream - S3-E3
Continuity mistake: When Kitty goes to get a midnight snack, her hair is down. But a few shots later, her hair is up in a ponytail.

X-Men: Evolution (2000)
Trivia: The store that Rogue and Wolverine are in when they are ambushed by the soldiers is the same store as the one that Wolverine showed up at in the first episode. The inside has changed, but the building and the scenery outside is still the same.

X-Men: Evolution (2000)
Continuity mistake: When Nightcrawler teleports into the police car, the seat belt is buckled around him. How did the seat belt buckle itself around him? The police wouldn't have buckled the seat belt for no reason.

X-Men: Evolution (2000)
Continuity mistake: In the breakfast scene, Nightcrawler teleports onto the chandelier to grab food. However, in previous shots we've seen of the dining room, there was no chandelier anywhere near the dining room table.

X-Men 2 (2003)
Continuity mistake: During the mansion invasion scene, Wolverine jumps from the balcony level. He drops onto one knee, and stabs two soldiers with his claws. He then stands up and throws them off. In the shot from behind once he's thrown them he's still part crouching, but in the very next shot from the front, he's fully upright. (00:40:10)

Unrealized Reality (1) - S4-E11
Visible crew/equipment: When Crichton is about to take his last journey into the wormhole (in this episode at least) he pulls down the visor of his helmet. We can see four square and round white lights reflected that are not from the current scene.

Charmed (1998)
Sleuthing With the Enemy - S3-E8
Revealing mistake: When Cole / Belthazar grabs Phoebe to use her as a hostage, you can see, very obviously, that he doesn't pull her. Phoebe actually steps in front of him willingly, despite the look of fear on her face.

Charmed (1998)
Sleuthing With the Enemy - S3-E8
Revealing mistake: When Phoebe throws the potion at Cole's coat, the vial doesn't shatter, even though it sounds like it. Watch closely, and you'll see the purple, unbroken vial bounce off the coat and away out of shot without any of the potion touching the coat.

Other mistake: When everyone is looking for Bobby, Matthew Lillard says that it is no use shouting because the glass is 100% sound proof. Yet when he is trapped in the cage, he hears Maggie screaming directions for him to dodge the ghost - not sound proof at all then.

Plot hole: Spike must have had his chip implanted while he was unconcious. If so, how did he attack The Initiative scientists without any pain when he tried to escape? You could argue that they programmed him to be capable of violence within The Initiative only, but why would super smart scientists program an evil being to be able to escape at the first opportunity? Joss Whedon has acknowledged this as an oversight.

Buffy The Vampire Slayer (1997)

Revealing mistake: Several times during Spike's escape from The Initiative, his reflection can be seen in the glass. Vampires aren't supposed to have reflections. (00:13:00)

Buffy The Vampire Slayer (1997)
Other mistake: We're shown a variety of photographs and pictures of different members of the gang at one point. One of those, the one of Xander and Anya, is actually a shot from the episode "Something Blue". I doubt very much that during the events in that episode, someone conveniently decided to take a photograph.

X-Men (2000)

Revealing mistake: In Toad's final scene of the movie, when he is beginning to fight Storm, he leaps into the air onto the balcony railing. If you look at the back of his belt, his wire is very, very obviously visible. There are several times within the movie when Toad or Storm has a wire showing, but none so obvious as that one.

Farscape (1999)
Factual error: Considering that the plant is supposed to secrete an acid that burns through Moya's entire outer hull, when we see the outer shot while the gas is igniting, there is a considerable lack of damage. The plant ate its way from outside the ship, directly into the ship's internal vents and tiers, yet from the outside, the ship looks exactly the same as always. Shouldn't it have shown some sort of erosion or hull breach where the plant entered the ship?
