Continuity mistake: In this episode, Leo tells Phoebe that white lighters are sent to their charges by "a group of elder white lighters known as the founders." Revealing mistake: Throughout the series, Finola Hughes appeared as Patty, the deceased mother of the Charmed Ones. Although Patty died in 1978, she sometimes appeared in flashbacks and time-travel episodes that took place in the mid to late 1970s, and other times she appeared as a ghost. However, over the eight years of the series, Finola Hughes aged noticeably. Patty was born in 1950, so she should always have been either a young woman in her mid 20s, or an unaging ghost having died at 28.

Character mistake: Katya (as Nina) says that Pandora's Box is "a gift the gods gave to Prometheus." But the Box was given to Epimetheus, not Prometheus. Epimetheus ignored the warnings of Prometheus, his brother, and accepted Pandora herself, who bore a box, as a gift from the gods.

Plot hole: The innocent who gets Piper's powers uses them to freeze Piper and Phoebe. But even though Piper and Phoebe no longer have their unique powers, they are still good witches; they were still able to scry, and we have encountered good witches on the show in the past who had no powers at all. And by the rules of magic within the show, good witches do not freeze.

Continuity mistake: Midway through the episode, the Seer is looking through the sisters' photo album. Several of the photos actually include everyone who was present for the events depicted, leaving nobody to take the photo. And since the events were shown in various episodes, we know that no photo was taken at those times by any automated method.

Someone to Witch Over Me - S7-E7
Continuity mistake: In episode 7-7: "Someone to Witch Over Me," Piper is killed by the demon Sarpedon's metallic claw. When the claw strikes her, she falls back with her arms behind her, landing partially on her palms, allowing her to complete the fall by sinking gradually to the ground. Then there is a cut to a closer shot, and we see her fall again. The second time, she falls directly into a more prone position with her arms raised and the backs of both hands facing the ground.

When Bad Warlocks Go Good - S1-E18
Continuity mistake: Prue finds Brendan, as a warlock, over the corpse of Father Austin. He runs away and she pursues him. He insists that he "didn't hurt Father Austin. [He] found him like that." Prue counters, "Before or after you turned into a warlock?" His response is, "After. When I saw what [my brothers] did to him, I was crazed. The rage turned me into a warlock." So doesn't that mean that he found Father Austin before he turned into a warlock, not after? (00:17:55)
Suggested correction: He said after because Brendan has always been a warlock. He was born that way.

Continuity mistake: When Prue describes the way the warlock Matthew traveled instantaneously, Melinda tells the sisters that it's called blinking, and that Matthew "must have copied it from another witch." But later in the series, every warlock encountered by the Charmed Ones had the ability to blink, and it was considered exclusively a warlock ability, meaning that no witch would have had it. Continuity mistake: In the teaser, the demon attacks Dr. Mitchell, killing him and also destroying his irises. His eyes turn white, and later dialogue confirms that the irises were gone from his eyes. However, when the Charmed Ones find his corpse, his eyes are open and his irises are visible. (00:01:10)

Then in this episode, she works at a bar and restaurant called "Quake," but she talks about Chef Moore quitting and leaving her in charge of the establishment, so it's not meant to be a different place.

Necromancing the Stone - S5-E21
Plot hole: Piper mentions that Grams banished the Necromancer "sixty years ago" at Patty's Wiccanning. Grams soon thereafter says that the Necromancer has had "almost seventy years" to regain his strength. However, Patty was born in 1950, and this episode aired on 5/4/03, at which time Patty's Wiccanning could not have been more than 53 years earlier.
