Hellraiser (1987)

Visible crew/equipment: As Frank is killing the last victim Julia brings him (the meek man), Kirsty slowly opens a door and slinks through. As she does, in the bottom left corner of the screen, you can see part of a shadow from either the camera or a cameraman. It's definitely a camera/cameraman shadow, since the shadow is wavering in a similar way to the camera's movement. (00:57:12)

Visible crew/equipment: When Julia sees the vision of Frank being tortured by the cenobites, at one point "Butterball" turns and looks at camera, and you can see a set-light reflected in his glasses for a brief moment.

Welcome To The Hellmouth (1) - S1-E1

Visible crew/equipment: When Buffy's mom drops her off at school at the beginning, watch closely when Buffy gets out of the car. In the passenger door's quarter glass window, you can see a reflection of a set-light (probably a fill light to eliminate shadows) for a few seconds when she opens the door.

Welcome To The Hellmouth (1) - S1-E1

Visible crew/equipment: In the beginning, when Buffy is having the prophetic dream, there's a shot right as it's about to end where the camera is above her and moving down closer to her. If you watch, you can blatantly see a shadow (more than likely the camera rig) grow over her as it gets closer and closer to her.

Van Helsing (2004)

Visible crew/equipment: When Dracula says "You can't kill me, Victor... I'm already dead!" in the beginning, pay attention. As the camera dramatically moves towards the two men, you can see the shadow of the camera appear on the lower half of Victor's face. It's not Dracula's shadow, because it appears after Dracula moves right in front of him, and perfectly coincides with the camera movement. There's also a brief flash of lightning, and the lightning shows that the shadow is very "boxy," unlike a human shadow.

Revolution - S1-E5

Visible crew/equipment: When Tara gets into her car after seeing her therapist with Max in the beginning of the episode, watch closely when the camera moves up to the side of Tara's car. If you look at the bottom-left corner of the screen, you can see the top of a crew-member's face and some camera equipment reflected in the driver's side window right before the scene changes.
