Many Happy Returns - S2-E4

Continuity mistake: During the fight between Anya and Clarke in front of the drop ship they are both covered in grime, and after Anya hits Clarke with the log Clarke's face becomes increasingly smeared with blood. Throughout the scene, you can see Clarke's face switch between grimy and bloodied at least a couple times, most noticeably in the shot just after Anya tackles her into the ground and the following one. Blood Must Have Blood: Part 2 - S2-E16

Revealing mistake: When Octavia storms into the lab where the Mountain Men have been holding the Ark's people, she has her sword in her right hand. As soon as Kane speaks she makes to sheath it, and we hear the sound we're used to expect to accompany that motion, but you can see she's just holding the unsheathed sword behind her. Then she proceeds to drop it, hiding part of the action from the camera with her body, and although you don't see her catch it again that is made evident from the way she eases the sword on the floor beside her right foot a moment later. Furthermore, in the next shot her sword is not in its sheath. (00:30:05)

Blood Must Have Blood: Part 2 - S2-E16

Continuity mistake: In the scene when Octavia and Maya enter the mess hall while trying to escape the guards chasing them, you can see Maya is walking in front of Octavia. In the shot immediately after, though, Octavia is the one in front and she is being stopped by a guard pointing a gun at her. In the following shot we are back to Maya walking in front still trying to cross the hall, just as she sees guards appear from around the corner; that is the point when the two turn around, and we are once again presented with the same shot of the guard intercepting Octavia, only from a different angle. Continuity mistake: When Clarke gets in Finn's tent with Raven to check on his wound you can see the cut running vertically on his chest, but during the whole previous episode the wound was horizontal. (00:06:35)

Contents Under Pressure - S1-E7

Continuity mistake: When Clarke extracts the knife from Finn's chest, suddenly they all get thrown to the ground. Once she stands up again, in the first shot she's holding the knife with the blade pointing behind her, as it was when she extracted it, but in the following shot the blade is pointing forward instead. Continuity mistake: When Clarke and Finn are undressing in the bunker, right after Clarke takes her shirt off in a closer shot they both have bare shoulders. In the following shot Finn has his shirt on once again before pulling it over his head. (00:39:30)

Revealing mistake: In the shot where Lash hauls Rosalind over the parapet to later drop her, you can see that he is actually lifting a mannequin as its legs very fluidly bend backwards while it is being dragged over. (00:32:00)

Trivia: In the scene at the airport where Mr. Gold, Emma and Henry are standing in line for the security check, the three children waiting right in front of them are Robert Carlyle's own children, acting as extras. (00:16:45)

Continuity mistake: When we see Emma giving Merida back her heart, shoving her away in the same motion, a couple of shots later the camera turns on Emma and we are shown her still doing the same movement, although several seconds have passed. (00:33:40)

Continuity mistake: When the lady comes to Dyson's gym to ask for his help, she writes her contacting info on the back of the picture she gives him. In the next shot, he's holding the same picture but the back is completely blank. (00:19:20)

Audio problem: During Kenzie's speech to Dyson at the gym, at one point when she asks "All of that just to give up now?" Dyson answers something, but there's no audio. (00:18:15)

The Woman - S1-E23

Continuity mistake: When Sherlock first accuses Irene of working for Moriarty, he pins her to a bookcase. When she shoves him away and breaks free, her hair ends up arranged with some strands in front of her. During the following minute, those strands repeatedly switch back and forth from behind her shoulders to in front of them. (00:38:25)

Continuity mistake: After Victoria touches the car and gets electrocuted she falls away, landing on her left side. In the next shot, though, a close-up of her face and upper torso is shown with her lying on her right side. Following that, we come back to a shot of the street with Victoria again lying on her left side. (00:41:30)

Hold on to Sixteen - S3-E8

Continuity mistake: During "Control", Blaine's jacket can be seen repeatedly switching from closed to open. Most noticeably when the chorus shouts "oh yeah" and he's seen high-fiving another member of the group on the right of the screen. This is shown two consecutive times from two slightly different angles. The First Time his jacket is open, the second time it got buttoned up. (00:28:25)

Heart of Darkness - S1-E16

Revealing mistake: At the end of the episode, when Emma goes to Gold to ask for his help, we can see he's wearing a wedding ring on his left hand in addition to the usual ring with the light blue stone he wears on the right. That's actually Robert Carlyle's own wedding ring; as he wrote during a twitter Q&A session, he forgot to take it off before they started to film. (00:40:35)

Visible crew/equipment: During "Valerie", as Santana resumes singing right after Mike has somersaulted over Brittany, you can see a cameraman filming the girls' group in the bottom left corner of the screen. (00:32:15)
