Just Lucky, I Guess - S2-E4

Continuity mistake: The bad guys are driving a dark green '62 Cadillac - with fins. After Five-O's sharp shooters fire at it, the Caddy hits a concrete wall. In the next shot of the wreck, the green '62 has turned into a black '64 - without fins. (00:42:45 - 00:44:35)

Monk (2002)

Factual error: When Richard Collier first arrives in 1912, the woman getting dressed in the hotel room is humming "You Made Me Love You," a song not published until 1913. It was first recorded by Al Jolson in June of 1913. The lady in the hotel shouldn't have known it yet. (00:30:00)

The Lonely - S1-E7

Continuity mistake: Allenby brings Corey a stack of paperback books. He's holding them with the back covers facing out until the shot cuts, and then the stack of books has reversed itself in his hand: the back covers are now facing inward. (00:20:30)

Forty Feet High and It Kills! - S2-E3

Revealing mistake: McGarrett and Victoria watch surfers from the beach. Thanks to the insertion of stock footage, they "see" surfers riding huge 6-foot waves. But in the shots of the actual locale behind them, the ocean is virtually flat. (00:09:30)

One for the Angels - S1-E2

Continuity mistake: As the hour is striking midnight, Maggie's toy robot turns itself on and walks past the alarm clock on her bureau. After a cut to Lou and Mr. Death, the scene returns to Maggie's room, and the robot walks past the clock again. (00:22:00)

Moonstone - S1-E24

Continuity mistake: Dr. Brice's name tag is partially tucked under her collar when she's inside the lab. When she walks into the corridor, the tag has shifted position and is outside the collar. Later, back in the lab, the tag keeps changing from over to under her collar repeatedly. (00:32:50 - 00:36:50)

Moonstone - S1-E24

Continuity mistake: General Stocker falls into a lunar quicksand pit and starts to sink. One arm disappears beneath the sand, but in the next shot, both his arms are outstretched and waving for help. (00:04:25)

Revealing mistake: At the end, when Guy loses the sword fight with Robin Hood, his body topples over a parapet and falls to the stone floor below. In the initial shot of him falling, it's a stunt man (who apparently broke his ankle in the process, according to the dvd commentary), but the next shot of the body landing makes the use of a dummy a little too obvious: the "corpse" bounces back up several inches before settling to the floor. To Hell with Babe Ruth - S2-E2

Continuity mistake: When Nagata breaks into the boarded-up house, he walks through a doorway full of cobwebs, covering his black suit with white dust and webs. But when the shot changes, his suit is suddenly perfectly clean again. (00:10:00)
