Curse of Chucky (2013)

Plot hole: When Chucky is decapitated, no blood comes out. He still has the scars from Bride/Seed of Chucky so that means he's still in the same doll's body. The longer he remains inside the doll the more human he becomes, so he should definitely bleed by this point in time.

Suggested correction: There's no telling exactly how his Voodoo works.

The writer/director even admitted this scene was a mistake. Prior films showed that once Chucky inhabited the doll for long enough, blood and organs would appear. The director simply chose to ignore this for this one scene, and has admitted it's a mistake.

Angel (1999)

Dad - S3-E10

Plot hole: When Angel is being pursued by the Lilliad demons, he is holding his son Connor and is talking to him in the car. Later on it is revealed that he is in fact holding a teddy bear with a bomb attached to it. If it wasn't his son, why was he talking to it? It couldn't be to fool the Lilliad demons because they wouldn't be able to hear him, as they were in separate vehicles.

Nosferatu (1922)

Plot hole: When Nosferatu is caught in the sunlight towards the end, his reflection can be seen in the tall standing mirror in the corner of the room. This makes no sense because vampires do not cast reflections. Deliberate or not, in the original Bram Stoker novel, there is a scene in which the character Harker is shaving, and is disturbed to see that the Count casts no reflection. There is also a passage that states, "He throws no shadow, he make in the mirror no reflect." Some would argue that the rising of the sun robs him of his powers, giving him the reflection. But his reflection is visible before the rooster starts crowing, before the sun even rises.

Suggested correction: That's all from the book, none of that is established in the film.

Plot hole: Live 50th anniversary episode, aired December 9th 2010 - When Sally is allowed into the rubble to comfort Molly as the firemen attempt to free her, one of them says they'll get Sally a hard hat. When we see her talking to Molly in the rubble, she has no hard hat on and instead one of the firemen is hilariously holding his hand above her head.

Primeval (2) - S4-E21

Plot hole: Willow hacks into The Initiative's computer system and discovers air ducts and electrical conduits leading to Adam behind 314, but the room doesn't show up on the schematic. It seems unlikely that nobody who didn't know about Adam would have noticed this and investigated.

Gone - S6-E11

Plot hole: For some strange reason, the guard at the arcade allows Buffy and Willow to leave the arcade with the invisibility ray-gun despite having absolutely no idea what it does.

School Hard - S2-E3

Graduation Day (1) - S3-E21

Plot hole: Healing Angel requires draining the blood of a Slayer. Buffy tries to kill Faith in order to get some of her blood but Faith gets away after Buffy stabs her at the end of "Graduation Day Part 1." The amount of blood on the dagger that she used wouldn't be enough blood to heal Angel, but it'd be a start. So why didn't Buffy just use some of the blood on the dagger? In the very first shot of "Graduation Day Part 2" we see the dagger coated with the stuff.

Castle (2009)

A Rose for Everafter - S2-E12

Plot hole: The security camera images of Castle kissing Kyra on the roof are completely inexplicable. The angle at which the images are taken are near enough identical to the angles at which the rooftop scene were shot. There were no pillars, poles or high walls for a security camera to be attached to at those particular angles, and certainly nowhere a photographer could hide, so where did the images come from?

Suggested correction: Beckett had Kyra followed, because she was a suspect. The photos were taken by the person following her.

Life Serial - S6-E5

Plot hole: In the 3rd time loop at the magic shop, the woman looking for the mummy hand looks surprised when Buffy knows what she wants. In the 2nd time loop, the woman tells Buffy that she called the shop 20 minutes earlier and was told that they had one. Therefore at least one of the shop's staff would be aware that a woman would be arriving looking for a mummy hand.

Something Blue - S4-E9

Plot hole: Before the opening credits where Buffy stakes the vampire in the cemetery, she stakes him below the left shoulder, above the heart. Yet he still turns to dust and dies.

Bad Girls - S3-E14

Plot hole: Buffy saves Angel from Balthazar by dropping a light into the tub of water that he's sitting in. But why would the tub have been put directly underneath a light? Balthazar was being kept alive throughout the episode, wouldn't having a light above him be a bit unwise?

THGhost

Helpless - S3-E12

Plot hole: When Buffy enters the house to rescue her mother she uses a stake to stop the front door from closing. A vampire then grabs it and the door closes. Why did Buffy use a stake knowing that there was a vampire there when she was almost killed by a vampire with her own stake earlier in the episode because Giles had been injecting her with a muscle relaxant?
