Factual error: During the surgical operation, Dr. Heiter makes incisions on the buttocks for the triangular flaps of connecting flesh; but the flaps are pointing outward, rather than inward. The only way to graft the buttock flesh to the face of the next subject would be with the buttock incisions pointing inward.
Continuity mistake: In the beginning of the movie, the doctor invites the two girls knocking on his door inside out of the rain because their car broke down. He gives them both water (laced with drugs). One of the girls knocks her glass over, spilling most of the water, which can be seen as the glass sits on the table. Then when the doctor leaves the room, while the two girls are talking, you can see that the glass on the table magically is filled again.
Revealing mistake: When Dr. Heiter takes Katsuro from his car, you see Heiter drag him out of the back seat. He then proceeds to lift Katsuro. If you watch Katsuro's hand, you'll see he "helps" Heiter by pushing himself on the door-frame of the car (you see his hand tense as he pushes himself up).
Continuity mistake: At the very beginning of the movie as the camera pans around to the parked silver Mercedes, the wheels are straight. However, as the camera enters the driver's window to show the Doctor, the steering wheel is turned to the left (note the control buttons and wood effect on the wheel are well out of line from where they would be if the car was going in a straight line).
Revealing mistake: When Doctor Haier is explaining the operation to the captives, he says he'll sever the ligament in the knee. However, around the 1-hour mark when the captives are walking by the pool, you can see the rear woman's leg raise her left foot off the floor when she moves forward, showing she still has control over her knee.
Suggested correction: He explicitly states that the ligament severed prevents the knee from being straightened, not bent.
Continuity mistake: In the first part when the guy is in the highway place parked, when the truck drives past, you can see the guy(who was looking at the pictures) is smoking. but in close ups he is not.
Revealing mistake: The victims are supposed to be surgically connected mouth-to-anus. The majority of the time, their heads are far too high to be connected this way.
Continuity mistake: Towards the end of the movie as the victims attempt the final escape, Katsuro uses the lamp to break the window. When he first picks up the lamp it has a shade and bulb in it. Once the next shot appears the shade and bulb are gone. (01:15:00 - 01:15:40)
