Revealing mistake: There are supposed to be two dead young Asian males sitting in the back of the car, but in this shot, it is so obvious that there are two dead 'dummies' riding in the back. Also, in the crash, a few seconds later, very blatant dummy heads are flying everywhere. (00:40:40)

Revealing mistake: Snipes drives a car into a diner, puts a bomb in the car and sets the accelerator so the car will back out of the diner with the bomb. As the car gets outside the bomb explodes causing the glass windows of the diner to blow out. You can see the wiring attached to each window, by the special effects company, making them blow out. Visible crew/equipment: When Wesley is in the interrogation room with the 2 way mirror, as the camera pans around the room the reflection of the camera and the crane it's mounted on, and even a crew member, are all perfectly visible in the mirror. (00:33:25)
