Continuity mistake: In Seth's trap, the mechanism that is connected to the hand crushers is moving one way and then stops. When it continues, it continues the same direction. However the hand crushers now go up. (00:04:30)
Continuity mistake: When Seth has been sliced by the blade, he spits blood that goes all over his face. It then cuts to Hoffman looking at him, then back to Seth, and now the blood on his face has gone. This happens three or four times. (00:05:10)
Continuity mistake: When Strahm looks at his phone while locked in the sick room, the signal bar is on the right and the battery signal on the left. When Hoffman plants Strahm's phone in his hideout to frame Strahm, the signal bar is now on the left and the battery on the right. (00:06:40)
Suggested correction: Battery is always on the right, signal is always on the left.
Continuity mistake: When Strahm has just discovered the hidden door near the start, the position of the drips from the glow-in-the-dark gunk changes between shots. (00:06:45)
Continuity mistake: When Hoffman has the orange blanket put on him by the paramedic after escaping the Gideon Building, she puts it on him twice. It is also at one point not covering his right shoulder, but in the next shot, when Fisk talks to him, it is. (00:11:00)
Continuity mistake: When Jigsaw's lawyer puts the opened envelope down on the desk, having taken the video out, he puts it horizontally on the folder. Camera cuts and it's now rotated 90 degrees instantly. (00:12:40)
Continuity mistake: When we first see Hoffman looking at the CCTV screens, we can see Ashley is lying down. When it shows her waking up, however, she is sat up. (00:20:20)
Continuity mistake: The amount of blood on Alison's padlock in the first of the group traps changes instantly. (00:22:00)
Continuity mistake: When the five victims are in collars ready to start their first test, Ashley gets decapitated at the end. There is a shot of her head on the ground in a pool of blood. But in another scene later on Mallick goes through the door into the next room where you can see the head on the ground, and the pool of blood under the head has vanished. (00:26:50 - 00:33:15)
Continuity mistake: When Ashley is decapitated, a pool of blood appears under her severed head. Later when we see the head from the second room, the pool of blood is not present. (00:26:50 - 00:33:30)
Continuity mistake: When Strahm looks at the file and says "got you" he points at the 4th record on the list. Camera cuts and now he's pointing at the 6th. (00:29:10)
Continuity mistake: When entering the second room, the position of the door, before it gets closed, changes depending on the camera angle. (00:33:15)
Continuity mistake: After the first room bombs go off, all the people are crouched on the floor and there is a load of dust. Camera shows Billy, then the people again. Now, there is no dust and everyone is stood up again. (00:35:05)
Continuity mistake: Just as the second room trap comes to a close we see 9 seconds on the timer and hear it ticking down. We hear 4 ticks and then there are 2 seconds left. There should be 5. (00:37:20)
Continuity mistake: The shotgun that is positioned to Hoffman's throat changes position between shots. Note the gap between the barrel and Hoffman's throat. (00:41:05)
Continuity mistake: Jigsaw puts the razor down on a piece of paper and it is in a near right angle. When Hoffman picks the razor up, it is still at an angle. However, as soon as it's in his hand it's perfectly straight. There is no time for Hoffman to straighten the razor without us seeing him do so. (00:47:00)
Continuity mistake: Throughout the movie, the clocks show incorrect time and at times changes from 8:20 to 18:20 and the countdown clock shows wrong sequencing. (01:01:00)
Continuity mistake: When Jill is talking to Erickson, the height of the key on her chest keeps changing. (01:03:10)
Continuity mistake: The air hoses attached to the glass box at the end of the film are hanging straight down in some shots, and are wrapped around themselves in others, without anybody touching them.
