Continuity mistake: At the very start we see Seth with the collar round his neck, connected by a bolt. When the collar opens at the end of the trap, the bolt has disappeared. (00:02:20)

Continuity mistake: In Seth's trap, the mechanism that is connected to the hand crushers is moving one way and then stops. When it continues, it continues the same direction. However the hand crushers now go up. (00:04:30)

Other mistake: Seth is told that in 30 seconds the pendulum will drop low enough to cut his body and in 60 seconds it will cut him in half. We then see the clock start ticking. After Seth has crushed his hands, we see the clock - 46 seconds have passed and yet the pendulum hasn't cut him yet. (00:04:35)

Continuity mistake: When Seth has been sliced by the blade, he spits blood that goes all over his face. It then cuts to Hoffman looking at him, then back to Seth, and now the blood on his face has gone. This happens three or four times. (00:05:10)

Continuity mistake: When Strahm looks at his phone while locked in the sick room, the signal bar is on the right and the battery signal on the left. When Hoffman plants Strahm's phone in his hideout to frame Strahm, the signal bar is now on the left and the battery on the right. (00:06:40)

Continuity mistake: When Strahm has just discovered the hidden door near the start, the position of the drips from the glow-in-the-dark gunk changes between shots. (00:06:45)

Revealing mistake: When Stahm is in the cube trap, there is one shot just before he sees his phone, gun, etc. and you can see a massive gap down the side of the cube where it wasn't shut properly. With the size of the hole, it would be extremely hard to keep water in the cube. (00:09:35)

Visible crew/equipment: When the police cars arrive after Strahm's cube trap, there is a very quick shot of the black police car passing before the camera. When it does, a white screen and black stage light are reflected on its surface. This is just before it shows the SWAT going in. (00:10:45)

Continuity mistake: When Hoffman has the orange blanket put on him by the paramedic after escaping the Gideon Building, she puts it on him twice. It is also at one point not covering his right shoulder, but in the next shot, when Fisk talks to him, it is. (00:11:00)

Other mistake: Virtually all the text in the various articles and reports in this movie are fake. Look at the John Kramer article at the very beginning, even at first glance you can tell it's an article about pilgrims or something and has nothing to do with John Kramer. Also, when Staham is going through the articles he found on Hoffman's sister, you can very plainly see that every article (there are a few) is just the same typed out story copied and pasted into various formats. (This doesn't even require slow motion, just pay attention.) (00:11:45)

Continuity mistake: When Jigsaw's lawyer puts the opened envelope down on the desk, having taken the video out, he puts it horizontally on the folder. Camera cuts and it's now rotated 90 degrees instantly. (00:12:40)

Continuity mistake: When we first see Hoffman looking at the CCTV screens, we can see Ashley is lying down. When it shows her waking up, however, she is sat up. (00:20:20)

Character mistake: When Erickson is telling Strahm that he no longer has any jurisdiction over the Jigsaw case, he mentions that over a dozen dead bodies were recovered at the meat packing plant, including two cops. There were in fact only 10 dead bodies: Danica Scott, Judge Halden, Timothy Young, Jeff Denlon, Lynn Denlon, Jigsaw, Amanda, Art Blank, Officer Rigg, and Detective Matthews. (00:20:28)

Continuity mistake: When the five victims are in collars ready to start their first test, Ashley gets decapitated at the end. There is a shot of her head on the ground in a pool of blood. But in another scene later on Mallick goes through the door into the next room where you can see the head on the ground, and the pool of blood under the head has vanished. (00:26:50 - 00:33:15)

Continuity mistake: When Ashley is decapitated, a pool of blood appears under her severed head. Later when we see the head from the second room, the pool of blood is not present. (00:26:50 - 00:33:30)

Other mistake: When Agent Strahm first starts investigating, he uses the FBI computer to look up Seth's name. He is heard typing many more characters, very quickly, than actually show up in the 'search' field on the screen. (00:29:20)

Trivia: When Agent Strahm is in his head box trap screaming, there is a lot of flash-cutting, including a shot of an outtake of the actor smiling with a towel in the cube and staff around him. Happens exactly at 9:52:370 into the movie, or frame 14203. VERY fast and easy to miss.

Question: It's explained that Hoffman wanted to be the hero and that he expected that no one would make it out alive in the building after Rigg's game. How in the world could he have expected or even decided this? He had no control over others who won their game in the building, so therefore how on earth could he predict that Strahm would kill Jeff in the previous film? And was the water cube intended to kill Strahm as he was warned not to proceed?

Answer: The games of Jigsaw and his followers were always intended to be extremely difficult, but with a small chance for survival based on the actions of the player. Hoffman probably wouldn't have cared either day if anyone else survived, but likely anticipated that most of the others would die. (Strahm's line about everyone being supposed to die with Hoffman being the soul survivor was more conjecture than anything.) In terms of killing Jeff- both Jeff and Strahm were emotional hot-heads, so in all likelihood one or both was going to die if they encountered each other. Strahm's water cube was intended to be an execution tool for breaking the rules. That's why Hoffman seemed so shocked that he miraculously survived.

Answer: Best guess is the person in the mask at the end of the tunnel would've killed Jeff and kept Strahm for the water trap? We could also assume that Hoffman has the same characteristics as Amanda and made the water trap "unwinnable" and just forgot to take Strahm's pen out of his pocket. And if Strahm never proceeded in the operating room, he would've died of suffocation?
