Visible crew/equipment: When Bond is just about to dive of the dam, the camera has a view from over his shoulder. You can just about make out over on the far left a car park with film crew vans and trucks. There is also much further down the dam a crane with a camera on. (00:01:35)
Visible crew/equipment: When Wade delivers the plane, he hands Bond a bag from Q and looks around. When he does a light screen is reflected in his glasses. (01:31:40)
Visible crew/equipment: Throughout the film, but in particular near the dish in Cuba, the models of buildings are obvious with typical small flames and/or droplets of water (from the days of 'Earthquake') when they explode. (01:40:00)
Visible crew/equipment: At the scene where Alec Trevelyan and Bond are fighting on the antenna cradle, there is one shot (about 1 second in length) where you can see the face of the stunt double. This occurs just after Bond rolls down the stairs, and we see a shot of Trevelyan putting a new magazine in his gun. The double jumps over the edge of the platform, and then Brosnan is in the room with the huge chain drive thing that moves the antenna. (01:53:00)
Visible crew/equipment: In the pre-title sequence there is a shot just after Bond leaves the Russian facility while escaping. In the shot, there is a plane moving and at the background Bond is standing and aiming his gun at the plane. There the person appearing isn't Brosnan, he is a stunt double. The stunt double's face is visible on blu ray. He's noticeable even without stopping the movie. (00:08:20)
Visible crew/equipment: At Q's lab, when Bond opens the car door a huge round stage light is reflected, which wans't there before in the previous opposite angle.
Chosen answer: The real answer, which I believe was explained better in the game or in special features. Regardless, I did hear, is that Ourumov wasn't aiming directly at Alec's head, but to the side and shot the ground behind him. But made it look like he shot him in the head. Alec would have felt the bullet whiz by him.

That makes more sense.

It has been a while since I watched the movie, and my copy is a VHS... but I believe if you watch very closely you can actually see the bullet hit the ground behind him with a tile breaking. I don't remember for sure if you can, but I seem to remember they did add that little effect.
