Continuity mistake: When Mother Firefly is showing Sheriff Wydell the next book from the pile, before she takes it the book on the table is closed, but when she takes the next one off the pile it is open. (00:37:35)
Continuity mistake: When Otis has taken Roy and Adam out to the desert, when they leave the both van doors are open, but in one shot one of the doors is shut. (00:41:25)
Continuity mistake: When Otis beats up Roy, his hat moves into a number of different positions between all the shots. (00:43:35)
Continuity mistake: When Sheriff Wydell hammers the nail through Otis's hand, at first the nail has gone through nearly all the way, but in the second shot it is sticking half way out. (01:29:05)

Continuity mistake: Wydell uses an axe and smashes out the windshield in Charlie's car. In the ending scene, it's intact again.
Continuity mistake: When Mother Firefly is in her cell, her hands are shackled with only a few inches of slack between the wrists. However, when she is telling Sheriff Wydell that he's got himself into something he's never getting out of, her hands are suddenly uncuffed, and she extends her arms near her head in a dramatic gesture.
