Continuity mistake: When Madeline is given the card with address of Lisle Von Rhoman on it, she tears it down the middle into two, and the two pieces she tears are torn neatly and straight down the middle. Yet later, when Madeline takes the two pieces of the card from her bag, they are torn much differently, and aren't torn straight and neatly. (00:19:45 - 00:28:20)

Continuity mistake: When Meryl Streep "blasts" Goldie Hawn with the shotgun and Goldie falls into the pool of water, she falls in backwards. However, when we see her get up, she is getting up from a face-down position. (01:05:25 - 01:07:10)

Continuity mistake: When Goldie awakens after being shot, she is soaking wet from having landed into a pool/pond. A couple minutes later, in the same scene, she is completely dry when fighting Meryl Streep... (01:07:45 - 01:09:10)

Continuity mistake: Goldie Hawn and Meryl Streep are fighting over Bruce Willis. Goldie Hawn is wearing a tight shirt and tight pants. Meryl is wearing a shirt with a skirt thingy attached and tight pants. Goldie has a hole through the middle of her abdomen where Streep has shot her earlier. They both grab shovels and start fighting. Bruce Willis starts to walk upstairs and he looks over the side and sees the shadows of the two women fighting. The hole is supposed to be on the shadow with the tight pants which is Goldie. Instead, the hole is shown on the body with the skirt attached to the shirt, which is Meryl. (01:10:20)

Revealing mistake: When Ernest runs away from Lisle, after he nearly drinks the live-forever potion, he runs down a hallway and commences to open a door with a doorknob on the left side of the door. It then cuts to the other side of the door, of Ernest walking through the door (into the party), and now the doorknob is also on the left side of the door, but it should be on the right side. Revealing mistake: Towards the end when Helen is about to fall down the stairs of the church (pulling Madeline with her) you can see the rope between her legs that is keeping her in the tipping point position. (01:36:55)

Continuity mistake: When Hellen kisses Madeleine at the party, the lip-mark is further to the right than it should have been, taken from the position of Hellen's head during the kiss.

Plot hole: Goldie Hawn's character obviously has lost a large piece of her spine when being shot, as seen through the hole in her body. So how does she keep her upper body up straight? When Meryl Streeps character broke her neck she couldn't keep her head up, so why can Hawn walk around as normal?

Continuity mistake: When Ernest is scared of seeing Madeline alive after dropping down the stairs the cord connecting the phone to the wall suddenly disappears (probably deliberate so he doesn't trip over it).

Continuity mistake: Lisle stabs the dagger on Madeleine's finger making her bleed, but in the next angle blood has disappeared from the dagger.

Sacha

Continuity mistake: At the book signing Helen kisses Madeline on the cheek and leaves a bright red lipstick mark. She appears to kiss her quite close to the ear but in the next shot, the lipstick mark is more in the center of her cheek.

Continuity mistake: After Ernest pushes Madeline down the stairs, he walks down to her and we see her laying down, with her legs in a 180 degree position. When Ernest talks on the phone to Helen, she moves her one leg, which is now directly in front of her. The camera has been on her the whole time in the background, so there is no way her leg could have been moved.

Revealing mistake: When Madeline is at Lisle's place the first time, she has to sit on a couch, next to a young man. Lisle tells him to make room for Madeline. He stands up and you can see a transmitter on his back attached to his pants.

Suggested correction: This is a security transmitter for the house.

Visible crew/equipment: During the scene when Meryl Streep takes the option, if you pay attention to the effects in the vial you see the top part of a man flash in and out as the lightning effect is happening.

Continuity mistake: Before Ernest lets go of the essence, the way he holds the bottle changes between the back and front shots.

Sacha

Continuity mistake: When Helen is shot in the tummy, the position of the lilies on the pond keeps changing between shots.

Sacha

Continuity mistake: After Madeline falls downs the stairs, her hair is messily over her face, but in the next angle it is neatly brushed. Also the position of her hand has changed.

Sacha

Continuity mistake: In the dressing room at the theatre, Madeline kisses Helen hello and leaves lipstick smear on her cheek. When the angle changes it's next to her jaw.

Sacha

Lisle: Drink that potion, and you'll never grow even one day older. Don't drink it, and continue to watch yourself rot.

Trivia: When Madeleine is in the morgue, we get a shot of a table and on that table is a jar with a brain and formaldehyde in it, and on the jar is a sign and the sign says "abnormal." This is a homage to the film "Young Frankenstein": Dr Frankenstein sent Igor to the morgue to get a specific brain, an intelligent man's brain, but Igor was startled and dropped the intelligent brain, so looked around for another one and he did: He found a brain with the label "abnormal" on it, and that is why the monster was so hard to teach and control.

Answer: Ernest painted Madeline's eyes before she came down the stairs. He even notes that the balance in her eyes were messed up.

Answer: Actually, they're both wearing coloured contacts. If you look at timestamp 01:17:23 when they say "Pleeeaase" to Ernest, you can see they're both wearing contacts. Meryl's aren't as noticeable, but they are the bluish-purple/periwinkle colour Ernest was using when he was painting her touch-ups on the pool table.

Answer: Her eyes changed when she fell into the hot tub. They were essentially boiled.

My interpretation was that when she was shot, she completely bled out, thus all color of her skin and eyes would fade away.

But there's no blood inside eyeballs?

There are several blood vessels that supply blood to the eyeball. The cornea is the only part of the body not supplied by blood.

It wasn't a hot tub she fell into. It was a little pond. No hot water.
