Continuity mistake: RJ got into his van and drove parallel to the house, almost reaching the end (right side) of the house when he saw Pearl in front of him and stopped. When RJ got out of his van, it was parked near the beginning (left side) of the house that he had already driven past. Also, Pearl was standing in front of a fence, so it didn't make sense that RJ would be driving straight toward a dead-end. (00:57:07)
Deliberate mistake: While walking bare-footed in the barn looking for RJ, Wayne does not raise his feet very high - that is, until he is about to tramp on the nail that is perpendicular to the floor. If Wayne had kept walking like he had been, he might have bumped into the nail or object holding it up, but not got punctured by the nail. (01:06:40)
Continuity mistake: After Pearl stabbed RJ (lying on the ground in front of his van) in the neck a few times, close-ups of the passenger side headlight showed blood splattering/spraying onto the headlight. Even after repeated spraying of blood onto the headlight nearly covering it in blood, front more distant views of the van did not show any noticeable blood on the headlight. The position of RJ on the ground also questions the angle at which the blood had to spray from his neck in order to land on the headlight. (00:59:57)
Answer: It was just the naked man chained up. Lorraine was being dragged inside when she gurgled, and the old man had a heart attack and died. They were just inside the front door.
