When the first Hårga hits him on the head, there is a visible bloody injury caused by the hit. How would they be able to keep him from bleeding to death? We see earlier in the film when they kill Josh that someone was wearing Mark's skin. That means they completely flayed him and then crudely reattached his skin in order to hang him from the ceiling later on.
