Other mistake: The police captain asks the leader of the SWAT team "are you familiar with the 18-storey building adjacent to the subject structure." The SWAT leader says yes sir. Shortly after, the SWAT leader rides a bike past the adjacent building. It's not 18 storeys, it's 5. (01:17:40 - 01:20:14)
Other mistake: When the Mayflower truck is on the move picking everyone up for the heist, a mysterious hole appears above the truck's cab, cut out in the letter "o" in "Mayflower" when Pop is picked up, that wasn't there a moment before, and then it vanishes. The same thing occurs after the heist, when the truck crashes into the police cars.
Continuity mistake: After Duke unlocks the gate, when the Mayflower moving truck is driven out of the lot there's a built-in tow hitch at the back of the truck, but when it pulls into the apartment building the tow hitch is gone - the back bumper is one solid piece.
Other mistake: After the Mayflower truck crashes into the police cars, the ramp is lowered at the back of the truck and the van quickly escapes down the ramp, but when it cuts to the wide shot of the action the van is not coming from the ramp's direction - it's on the street to the right of the truck and its ramp.
Continuity mistake: When the Mayflower moving truck leaves the lot and picks up the crew there's no license plate on the front bumper when Tommy gets in, but when the truck crosses Broadway suddenly there's a license plate on the bumper, and then it vanishes. Additionally, when the van's stowed away in the truck it doesn't have an antenna on the hood and "256-245" is the van's license plate number, but after the heist there's an antenna and the van's plate number is "C47-618".
Continuity mistake: After Mrs. Longene eggs on Socks to kill her husband for not revealing the safe's combination to save her, it cuts to the first man on Everson's team rappelling without gloves - the gloves are in his back pocket, and in the next shot he's wearing the gloves. Then the next cop to rappel across is not Everson, but in the close-up it's Everson reacting to the rope burns on his hands as if he had just crossed.
Continuity mistake: After Duke gets frustrated that Mr. Longene still hasn't given him the combination to the safe he asks for Socks, and when it cuts to Everson and his team rappelling to another building, as Everson climbs over the railing his shoes have changed to climbing boots and his badge has vanished.
Visible crew/equipment: When Duke gets into the limousine with Angelo for the funeral procession, the reflection of the camera and a crew member can be seen on the open door's window.
Other mistake: During the heist the Mayflower truck is parked at the building's 91st Street entrance, and the police have blockaded 5th Ave at the corners of E. 90th and 92nd Streets. When the guys try to flee the truck is driven down 91st so as to turn onto 5th Ave, and after the driver's close-up there's an overhead wideshot of 5th Ave and the truck should be coming from 91st, but it's actually coming from further up - towards 92nd Street, with the corner of 91st in front of it, which is impossible since it should have made the turn at 91st.
Revealing mistake: After Duke hears noise coming from the stairwell he locks the doors at the end of the hallway. After Duke shoots Parelli it cuts to the police firing at those locked doors, and when the doorknob blows off we can see that it's only a glued on doorknob, with no hole in the wood door, so it's impossible to have actually locked it.
Visible crew/equipment: After Duke unlocks the padlocked gate, when the Mayflower truck is driven out of the parking lot the reflection of crew members, location lighting, and a reflector screen can be seen on the surface of the truck.
Visible crew/equipment: When the Mayflower truck crashes into the police cars, crew members and the second camera (presumably filming the stunt from the overhead angle) can be seen in the wideshot at the left side of the screen, before the camera pans to the right for a close-up of squad car 351.
Visible crew/equipment: At the start, when the prison psychologist asks, "What is love," the reflection of two crew members with the camera between them, and the window behind them, can be seen on the pictures' glass hanging on the wall.
