Pulp Fiction

Visible crew/equipment: When Vincent enters Mia's home for the first time, a camera and a camera operator are visible in a reflection on the windows in front of them. They have a black blanket or tarp covering them to make them harder to see. Also, a crew member on the far right sneaks in at the very last second. (00:30:50)

Visible crew/equipment: When Vince and Mia get back from Jack Rabbit Slims, and Mia is putting on the music, the shadow of a person walks past her on her left, but Vince is standing still in that scene. (00:48:35)

Suggested correction: I've viewed this mistake many times. What really happened here was Vince actually turned and walked down the hall to take the piss and what we are seeing is HIS shadow as he walks toward an over-lit backdrop. Then to make it worse the producers place him right back at the spot with his keys in hand and a softer lighting to make it look more romantic. So the real mistake could be when they made that sudden change without reviewing enough of the previous entry scene. (Either way you can still barely see how he turns to his right before walking casually down the hall).

Visible crew/equipment: When Jules is talking to Marcellus and describing what would happen if Jimmy's wife came home and found them all with a dead body, we see what would possibly happen in a scene where the camera follows the wife to where they are carrying the dead body. You can sometimes see the camera shadow on the walls when the camera follows her. (01:55:45)

Visible crew/equipment: Near the end of the movie as Ringo is going to the kitchen of the cafe to let out all the waiters and the kitchen staff, the shadow of the camera is visible on the right side near the door while Ringo opens it. (02:18:24)

Visible crew/equipment: When Butch and Marcellus are tied up and Zed is interrogating them, you can sometimes see the shadow of the stick of the boom mike on Zed, and sometimes even the microphone part of it.

Visible crew/equipment: Towards the end of the "Divine Intervention" scene, the shadow of a boom mic can be seen above Jules' head in the top left corner of the screen when he says "Wrong! Wrong, this shit doesn't just happen".

Visible crew/equipment: When Vincent is talking to himself in the bathroom, just before Mia's overdose, the shadow of the boom mic is reflected in the mirror next to him.

Visible crew/equipment: When Bruce Willis pulls up to the Motel on Grace, not only can you see a reflection of a boom mic, you can also see a crew member turn around and run out of the reflection.

Deliberate mistake: Both in the first scene and last scene of the movie, we see Yolanda and Ringo starting the robbery by jumping from their seat and start threatening the costumers. In the first scene, Yolanda says "And I'll execute every motherfucking last one of you." But in the last scene the line changes to "and I'll execute every one of you motherfuckers". Suggested correction: Tarantino has explained that this is not an error, rather, he did this on purpose. When we first examine the scene, we are seeing Ringo and Yolanda's conversation from their perspective. Obviously, because this is their conversation, what we hear first is probably what was actually said. However, at the end of the film, what is said is different because we are no longer viewing the situation from Ringo and Yolanda's perspective, but rather everyone else in the diner, most specifically Jules.

Sounds like a typical Tarantino excuse for a stupid blunder.

I believe about 70% of the so-called mistakes called out here are really cinematic techniques to show points of view, highlight significant action, and underscore dramatic moments.
