Continuity mistake: As the US Embassy in Caracas, Venezuela is being evacuated, the ambassador steps into a Black Hawk helicopter with markings 33 and 3016. After her Black Hawk arrives at the Aeropuerto Francisco de Miranda Airport in Caracas, it has markings 60 and 2065. (00:25:48 - 00:29:20)
Character mistake: One character says a concentration of 0.09 parts per million in the water is bad: "if it were salt it'd taste like you were drowning in the ocean." Rubbish. The concentration of salt in seawater is about 35,000 parts per million.
Revelations - S2-E8
Continuity mistake: Blanca mentions that she has known for "two years" that she is HIV-positive. Blanca receives her diagnosis in the first episode of the series, but the story has a two-year gap between the first two seasons. So, in-story, Blanca has known her status for four years.
Other mistake: The shopkeeper hands Danny a Beretta 92fs/M9 handgun with a detached suppressor. To mount a suppressor to the muzzle end of the handgun the barrel needs to be threaded, and this one isn't. Sometimes barrels are threaded and when no suppressor is attached they are protected by a thread barrel cap to protect the threads from dings and bumps. There is no thread cap here either. (00:22:35)
Black Blood - S1-E1
Other mistake: All the Greek name variants are used over the Latin ones - including even "Odysseus" over the more familiar "Ulysses" (who is the same person). However when the story of Actaeon comes up, it's suddenly the goddess "Diana" that he saw. But all the gods have their Greek names, so that should be "Artemis." (As indeed is used in episode 2). Character mistake: When the bodyguards enter elevators, they always enter last; they should be entering first to ensure it is secure. This is particularly visible in the hotel where the elevator can stop on any floor and Special Protection Branch does not have complete control of it as an express.
The Book of Occupation: Chapter Five: Requiem for Tavon - S3-E5
Visible crew/equipment: Right before Anissa and Tavon are attacked by Khalil, the shadow of a camera can be seen on the ground in the bottom right corner of the screen as they approach the path.
Chapter Twenty-Eight: Sabrina Is Legend - S3-E8
Visible crew/equipment: During the strobe lighting at the very end of series three, with Faustus Blackwood seemingly releasing something evil, it is noticed that during the flashes, it is leaving after effects, showing the shape of the equipment that make the strobe lighting. (00:58:00 - 01:00:00)
Coffee Quest/Phoenix Rises - S1-E18
Other mistake: In Phoenix Rises, when Phoenix runs away, the Big Coffee sign is missing.
Character mistake: The invitation for the wedding comes with a calendar for the month of April, with the date of the wedding circled. There are 31 days shown on the calendar, but there are only 30 days in April. (01:12:42)

Fear and Loathing in Beverly Hills - S2-E8
Continuity mistake: In one part of the scene where Joe has Delilah's iPhone and it's ringing, it shows the call with a green answer button and a red decline button, then, it shows it only with a slide to answer option. (00:53:00 - 00:59:00)
Continuity mistake: When we see some parked cars, a silver car is parked on the white line, then within the white lines. (00:18:57)
Continuity mistake: John Paul Getty approaches the kidnapper's cars and Getty removes his sunglasses and tucks them into his coat pocket. The next shot shows him still wearing them as the kidnappers approach.
Continuity mistake: At breakfast the apple being cut for Camille as she poses a question to Adora is suddenly on her plate as the scene cuts to Adora's immediate reaction. (00:05:07)
Audio problem: The kids are sitting across from an older couple in a diner. The older woman is talking about her husband. The scene cuts to a shot behind the old woman. Her mouth isn't moving but she is talking.
Factual error: Sir John Franklin, Francis Crozier and James Fitzjames all wear a crown over an anchor on their epaulettes, the rank badge for a captain with over three years in rank. This is correct for Franklin and Crozier, but Fitzjames only held the rank of commander, and should therefore only be wearing an anchor on his epaulettes.
Factual error: After Dr. Austin directs the anesthesiologist to urgently start phentolamine intravenously to address the patient's hypertensive crisis on the operating room table, the anesthesiologist is seen quickly rolling the clamp on the line, purportedly to make it run faster. The clamp however is being rolled in the direction to close off line's flow. (00:20:20)
